Abnormal menstrual bleeding
Hello Dr.
Lu: I would like to ask you about some abnormal menstrual symptoms and physical conditions I have experienced in the past few months.
My last menstrual period started around August 20 and ended around August 25.
After that, I took emergency contraception at the end of August.
In early September, I experienced a small amount of spotting that lasted for about five days.
On September 20, I had a normal menstrual period that also lasted about five days.
All of these instances felt normal, and I did not experience any significant discomfort.
My period ended on September 25, after which I had unprotected sexual intercourse with ejaculation inside.
My next period started on October 17, and this time the flow was lighter but lasted for five days, with no noticeable abnormalities.
My period ended on October 21, and I had unprotected sexual intercourse again with ejaculation inside.
In November, about a week before my expected period, I began to feel significant dizziness and experienced nausea when smelling oil or food.
My period started on November 22, but the flow was very light; I barely saw any blood on a regular sanitary pad throughout the day, although there was some spotting when I used the restroom.
I still felt nauseous when smelling food or oil.
I would like to know if this situation could indicate a possible pregnancy.
Yi, 20~29 year old female. Ask Date: 2015/11/23
Dr. Lü Lizheng reply Obstetrics and Gynecology

It sounds like you have been experiencing some unusual menstrual patterns and symptoms that have raised concerns about a potential pregnancy. Let's break down your situation and address your questions regarding abnormal menstrual bleeding and the possibility of pregnancy.
First, it's important to understand that menstrual irregularities can occur for a variety of reasons, including hormonal fluctuations, stress, changes in weight, and even the use of emergency contraception, such as the morning-after pill you mentioned taking in late August. This medication can disrupt your normal menstrual cycle and may lead to lighter or irregular bleeding in the following months.
From your description, it seems that you have had several instances of bleeding that differ from your typical menstrual flow. The key points to consider are:
1. Timing of Bleeding: You mentioned having a normal menstrual period in September and October, but the bleeding in November was significantly lighter than usual. This could be a sign of implantation bleeding, which occurs when a fertilized egg attaches to the uterine lining, typically around the time your period would be expected. However, it can also be attributed to other factors, such as hormonal changes or the effects of the emergency contraception.
2. Symptoms of Pregnancy: The symptoms you described, such as dizziness and nausea when smelling certain foods, are common early signs of pregnancy. These symptoms can occur as early as a week or two after conception due to hormonal changes in your body. However, they can also be caused by other factors, including stress or dietary changes.
3. Testing for Pregnancy: Given that you have had unprotected sexual intercourse and are experiencing unusual symptoms, it would be prudent to take a home pregnancy test. These tests are most accurate when taken after a missed period, but they can sometimes detect pregnancy a few days before your expected period. If the test is positive, it would confirm pregnancy, and you should schedule an appointment with your healthcare provider for further evaluation and prenatal care.
4. Consulting a Healthcare Provider: Regardless of the outcome of the pregnancy test, it is advisable to consult with a healthcare provider about your menstrual irregularities and symptoms. They can perform a thorough evaluation, including a physical examination and possibly blood tests, to determine the cause of your symptoms and bleeding patterns.
5. Understanding Abnormal Bleeding: Abnormal menstrual bleeding can indicate various conditions, including hormonal imbalances, pregnancy, or other underlying health issues. It is essential to monitor your symptoms and seek medical advice if you experience significant changes in your menstrual cycle or if you have concerns about your reproductive health.
In summary, while your symptoms could suggest a possibility of pregnancy, they could also be attributed to other factors. Taking a home pregnancy test and consulting with a healthcare provider will provide you with clarity and help you understand your situation better. Remember, early prenatal care is crucial for a healthy pregnancy, so don't hesitate to reach out for professional guidance.
