Difficulty in completely emptying the bladder and some issues with sexual function?
Hello, Dr.
Hsu.
I started having a habit of masturbation around middle school, almost once a day for about 10 years now (recently I have been moderating it).
Recently, I have encountered some issues:
1.
I feel like I can't completely empty my bladder when urinating, and the flow of urine doesn't feel very strong.
At the end of urination, I have to contract my anal sphincter to expel the remaining urine.
Is this something that typically happens to others?
2.
A few minutes after urinating, I feel the urge to urinate again, but the sensation seems to originate from the urethra, somewhat similar to the feeling of ejaculation.
If I touch my genitals, the sensation becomes more pronounced, but it's not an urgent desire.
When I try to go to the bathroom, I can't seem to urinate, and I have to stand for a long time while continuously contracting my pelvic floor muscles to release a little bit.
3.
These symptoms occur in the morning but become particularly noticeable in the evening.
Even after urinating before showering, I find that when I squat down to wash my hair, I end up urinating again, and it's not just a few drops, but it’s not a large amount either.
4.
After urinating before bed, the urge to urinate becomes more pronounced when I lie down, but after about 10 minutes, the urge subsides.
However, I've noticed that for the past month, I have been experiencing a strange pulling sensation on the right side of my abdomen (around the kidney area) when I urinate, stretch, or get up.
It’s not exactly painful, just an odd feeling.
Could this be related to urine reflux into the kidneys? I have consulted a doctor, and urine tests showed no signs of infection.
An ultrasound of the bladder indicated normal residual urine, but I still feel a sensation of residual urine in the urethra.
This doesn’t seem like frequent urination because I don’t feel an urgent need; it’s just a sensation of needing to urinate.
In recent months, due to the approaching national exams, I have been quite anxious, and I don’t wake up to use the bathroom at night.
The doctor suggested that this might be due to anxiety.
Is that true? But if this continues, could urine reflux lead to kidney issues?
Regarding sexual function: I can achieve an erection normally, but it’s not as firm as a cucumber.
If I bend my penis, it can be slightly bent.
My stamina is also poor; I lose my erection quickly if I get distracted and need continuous stimulation to maintain it.
There are many articles online claiming that masturbation can cause premature ejaculation and erectile dysfunction.
Is this true? I feel that my sexual function is not as good as it used to be.
After 10 years of masturbation, could this be causing these issues? Thank you.

Dr. Xu Weikai reply Urology
Mr.
Cia: Hello, regarding your urination issues, you can undergo urodynamic testing to identify which aspect may be problematic.
Note: Since you have already undergone several tests with no issues, it may be worthwhile to examine the neuromuscular system.
Additionally, it could be a case of autonomic nervous system dysfunction, which may require regular exercise to alleviate.
Masturbation can indeed lead to premature ejaculation, primarily due to the short duration typically associated with it, which becomes a habit.
However, you can gradually improve this through accumulated experiences from sexual intercourse.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

More Info
Hello! Thank you for sharing your concerns regarding urinary issues and sexual function. It's important to address these matters comprehensively, as they can significantly impact your quality of life. Let's break down your symptoms and questions to provide a clearer understanding.
Urinary Symptoms
1. Incomplete Urination and Weak Stream: The sensation of not fully emptying your bladder and having a weak urine stream can be indicative of several issues. Common causes include bladder outlet obstruction, which may arise from conditions such as benign prostatic hyperplasia (BPH) or urethral stricture. However, given your age and the absence of significant findings in your ultrasound, these may be less likely. It could also be related to pelvic floor dysfunction, where the muscles that control urination do not function optimally.
2. Frequent Urgency and Sensation of Urination: The feeling of urgency shortly after urination, especially if it feels like it originates from the urethra, can be frustrating. This could be due to a hypersensitive bladder or pelvic floor dysfunction. Stress and anxiety can exacerbate these sensations, leading to a cycle of discomfort and urgency.
3. Postural Effects on Urination: The observation that you experience increased urgency when bending down or in certain positions may suggest that your bladder is more sensitive to pressure changes. This could also be linked to pelvic floor muscle tension, which can be affected by stress and anxiety.
4. Pain in the Right Side: The discomfort you describe in the right side of your abdomen, particularly near the kidney area, could be related to several factors. While it’s less likely to be due to urine reflux, it could be associated with muscle tension or even referred pain from pelvic floor issues. If this pain persists, further evaluation may be warranted to rule out any underlying conditions.
Psychological Factors
You mentioned that you have been under stress due to upcoming exams. Stress and anxiety can significantly impact both urinary function and sexual performance. The body’s response to stress can lead to increased muscle tension, which may affect the pelvic floor and bladder function. It’s important to manage stress through relaxation techniques, exercise, or counseling.
Sexual Function Concerns
1. Erection Quality and Duration: You noted that while you can achieve an erection, it may not be as firm as you desire, and you experience difficulty maintaining it. This could be related to psychological factors, such as anxiety or performance pressure, especially if you find that you need continuous stimulation to maintain an erection.
2. Masturbation and Sexual Health: The concern that masturbation could lead to erectile dysfunction or premature ejaculation is a common one. However, research generally indicates that masturbation is a normal sexual activity and does not inherently cause these issues. That said, if it becomes compulsive or interferes with your sexual relationships, it may be worth exploring your habits and their impact on your sexual health.
Recommendations
1. Consult a Urologist: Given your symptoms, it would be beneficial to see a urologist who can perform a thorough evaluation, including possibly a urodynamic study to assess bladder function and pelvic floor dynamics.
2. Pelvic Floor Therapy: If pelvic floor dysfunction is suspected, physical therapy focusing on the pelvic floor can be very effective in addressing urinary symptoms and improving sexual function.
3. Stress Management: Incorporating stress management techniques such as mindfulness, yoga, or cognitive-behavioral therapy can help alleviate both urinary and sexual function concerns.
4. Lifestyle Modifications: Ensure adequate hydration, maintain a balanced diet, and engage in regular physical activity to support overall health.
5. Open Communication: If you are in a relationship, discussing your concerns with your partner can help alleviate performance anxiety and foster a supportive environment.
In conclusion, while your symptoms may be concerning, many factors can contribute to urinary and sexual function issues. A comprehensive approach involving medical evaluation, lifestyle changes, and stress management can lead to significant improvements. Thank you for sharing your experiences, and I hope this information helps guide you toward a resolution.
