Red spots on the glans penis?
Hello, Doctor Hsu.
I would like to ask you about something.
After I masturbated yesterday, I felt some discomfort in the glans, but I didn't think much of it.
However, this morning when I went to the bathroom, I noticed many small red spots on the glans, and the foreskin is also red.
Sometimes, I even feel a slight stinging sensation.
Normally, my foreskin covers the glans, but I do retract it to wash during showers.
Could this be related? I discussed this with a close friend, and he mentioned that he had a similar situation, which worsened to the point of ulceration a few days later.
He went to see a doctor who prescribed him zinc oxide ointment, so I am currently using his ointment, but I am worried it might get worse.
Do you have any recommendations, Doctor? Thank you.

Hello! Thank you for reaching out with your concerns regarding the red spots on the glans (the head of the penis) and the associated discomfort. It’s important to address these symptoms carefully, as they can indicate various underlying conditions.
Firstly, the appearance of red spots on the glans, along with redness of the foreskin and occasional stinging sensations, can be attributed to several potential causes. One common reason could be irritation or inflammation, which may arise from friction during sexual activity or masturbation, especially if there was a lack of lubrication. This can lead to a condition known as balanitis, which is inflammation of the glans. Balanitis can manifest as redness, swelling, and discomfort, and it may sometimes be accompanied by small red spots or lesions.
Another possibility is a yeast infection, particularly if you have a history of such infections or if you have been in contact with someone who has one. Yeast infections can cause similar symptoms, including redness, itching, and discomfort. Additionally, sexually transmitted infections (STIs) such as herpes or gonorrhea can also present with red spots or lesions on the glans, along with other symptoms like pain or discharge.
Regarding your hygiene practices, it’s good that you retract the foreskin during bathing to clean the area. However, if you are not doing this regularly, it can lead to a buildup of smegma (a combination of dead skin cells and oils), which can irritate the glans and foreskin. Maintaining proper hygiene is crucial in preventing infections and irritations.
As for the use of zinc oxide ointment, while it can provide a protective barrier and may help soothe irritated skin, it is essential to ensure that the underlying cause of your symptoms is properly diagnosed. Using a friend’s medication without consulting a healthcare professional may not be advisable, as the treatment should be tailored to the specific condition you are experiencing.
Here are some recommendations for managing your symptoms:
1. Avoid Irritation: Refrain from any sexual activity until the symptoms have resolved. This will help prevent further irritation and allow the area to heal.
2. Gentle Cleaning: Continue to clean the area gently with mild soap and water. Ensure that you rinse thoroughly and dry the area completely.
3. Topical Treatments: If you choose to use a topical treatment, consider using an over-the-counter antifungal cream if you suspect a yeast infection, but it’s best to consult a healthcare provider first.
4. Hydration and Lubrication: If friction is a concern, using a water-based lubricant during sexual activity can help reduce irritation.
5. Consult a Healthcare Provider: Given the symptoms you described, it would be wise to schedule an appointment with a healthcare provider or a urologist. They can perform a physical examination, possibly take a swab for testing, and provide a definitive diagnosis and appropriate treatment.
6. Monitor Symptoms: Keep an eye on your symptoms. If they worsen, or if you develop additional symptoms such as fever, discharge, or severe pain, seek medical attention promptly.
In conclusion, while it’s understandable to feel concerned about the red spots and discomfort you are experiencing, proper diagnosis and treatment are crucial. Avoid self-medicating with someone else's prescription, and seek professional advice to ensure you receive the appropriate care for your condition. Take care, and I hope you find relief soon!
