Rubber band ligation for internal hemorrhoids?

Hello Doctor, I underwent a hemorrhoid ligation procedure on December 25th, and it has been 11 days since the surgery.
However, I feel like there has been no improvement; I constantly feel a sense of fullness and have a persistent urge to have a bowel movement (though I don’t actually need to go).
Is this normal? The doctor mentioned that this sensation would likely improve after about a week, and that I might experience some discharge and slight bleeding.
However, due to the constant urge, I find myself going to the bathroom frequently.
Now, I feel that my external hemorrhoid has worsened (it has become larger) and there is some pain (it feels like it’s broken skin).
What should I do? I’m feeling quite distressed! Thank you, Doctor.

If the ligation of internal hemorrhoids is performed too close to the anal opening, this phenomenon may occur.
Generally, the symptoms will last for one to two weeks (while normal ligation typically lasts for three days).
During this time, only oral anti-inflammatory medications and warm sitz baths can be used to alleviate discomfort.
Topical ointments and rectal suppositories may also be helpful.
The symptoms should be nearing resolution, so please be patient with the treatment and avoid constipation.

After undergoing hemorrhoid ligation surgery, it is not uncommon for patients to experience a range of symptoms during the recovery period. The concerns you are expressing, such as a feeling of fullness, persistent urge to have a bowel movement, and discomfort around the surgical site, can be part of the normal healing process. However, it is essential to differentiate between typical post-operative symptoms and those that may indicate complications.

Typically, after hemorrhoid ligation, patients may experience some discomfort, swelling, and changes in bowel habits. The sensation of fullness or an urge to defecate can occur as the body adjusts to the changes made during surgery. This feeling may persist for a week or two as the tissues heal. It is also common to have some discharge and minor bleeding, especially if there is irritation or if the ligated hemorrhoids are healing.

However, if you are experiencing increased pain, significant swelling, or if the external hemorrhoid appears to be worsening (becoming larger or more painful), it is crucial to consult your healthcare provider. These symptoms could indicate a potential complication, such as thrombosis of the external hemorrhoid or an infection.
Here are some steps you can take to manage your symptoms and promote healing:
1. Pain Management: Over-the-counter pain relievers, such as acetaminophen or ibuprofen, can help alleviate discomfort. Always follow the dosage instructions and consult your doctor if you have any concerns about medication interactions.

2. Dietary Adjustments: To prevent constipation and ease bowel movements, increase your fiber intake through fruits, vegetables, and whole grains. Staying hydrated by drinking plenty of water is also essential.

3. Warm Baths: Soaking in a warm bath (sitz bath) can help relieve discomfort and promote blood flow to the area, aiding in healing.

4. Avoid Straining: Try to avoid straining during bowel movements, as this can exacerbate pain and prolong recovery. If you feel the urge to go, respond to it, but do not force it.

5. Follow-Up Care: Since you are experiencing ongoing symptoms, it is advisable to follow up with your surgeon or healthcare provider. They can assess your condition, provide reassurance, and determine if any additional treatment is necessary.

6. Monitor Symptoms: Keep track of your symptoms, including any changes in pain levels, bleeding, or new symptoms. This information will be helpful for your healthcare provider during your follow-up visit.

In summary, while some discomfort and unusual sensations can be expected after hemorrhoid ligation, persistent or worsening symptoms warrant further evaluation. Your healthcare provider is the best resource for addressing your concerns and ensuring a smooth recovery. Don't hesitate to reach out to them for guidance and support.
