Sudden bleeding and pain in the lower body?

Hello, doctor.
Recently, when I was having sexual intercourse with my girlfriend, I didn't use a condom, but we practiced withdrawal.
This week is supposed to be when her menstrual period arrives.
Today, when she went to the bathroom, she suddenly experienced bleeding and felt pain in her lower abdomen.
However, there was no bleeding when she wasn't using the bathroom, but she has been feeling pain continuously (we did not have intercourse yesterday).
What could be the reason for this?

Xiao Ma, 20~29 year old female. Ask Date: 2016/01/15

Dr. Lü Lizheng reply Obstetrics and Gynecology

The pain in the perineal area may be due to inflammation.
Please seek medical examination and treatment.
Sincerely, Dr.

Sudden bleeding and pain in the lower body, especially in the context of recent sexual activity, can be concerning and may have several potential causes. It’s important to approach this situation with care and consider various factors that could be contributing to these symptoms.

First, it’s essential to understand that the timing of the bleeding coinciding with your girlfriend's menstrual cycle could indicate that she is experiencing her period. Many women experience varying degrees of discomfort or pain during menstruation, known as dysmenorrhea. This pain can sometimes be accompanied by unexpected bleeding, especially if the menstrual cycle is irregular or if there are underlying conditions affecting her reproductive health.

However, if the bleeding is significantly different from her usual menstrual flow or if the pain is severe, it could suggest other issues. Here are some potential causes to consider:
1. Menstrual Cycle Irregularities: If your girlfriend's menstrual cycle is irregular, she may experience unexpected bleeding. Hormonal fluctuations can lead to spotting or breakthrough bleeding, which can occur outside of the expected menstrual period.

2. Infection: Sexually transmitted infections (STIs) or urinary tract infections (UTIs) can cause pain and bleeding. Symptoms of an infection may include unusual discharge, burning during urination, or fever. If there is any suspicion of an infection, it is crucial to seek medical attention.

3. Cervical or Vaginal Trauma: Engaging in sexual activity without adequate lubrication or with excessive force can sometimes lead to minor injuries in the vaginal or cervical area, resulting in bleeding and pain. This is particularly relevant if your girlfriend has not been sexually active for a while or if there was any discomfort during intercourse.

4. Ovarian Cysts: Sometimes, ovarian cysts can cause pain and irregular bleeding. If a cyst ruptures, it can lead to sudden pain and bleeding, which may require medical evaluation.

5. Ectopic Pregnancy: Although less common, if there is a possibility of pregnancy, an ectopic pregnancy (where a fertilized egg implants outside the uterus) could cause bleeding and pain. This is a medical emergency and requires immediate attention.

Given these possibilities, it is crucial for your girlfriend to monitor her symptoms closely. If the bleeding continues, becomes heavier, or if the pain worsens, she should seek medical attention promptly. A healthcare provider can perform a thorough evaluation, which may include a physical examination, pelvic exam, and possibly imaging studies or lab tests to determine the cause of the bleeding and pain.

In the meantime, it may be helpful for her to rest, stay hydrated, and use over-the-counter pain relief if necessary, provided she has no contraindications to such medications. It’s also important to communicate openly about any concerns regarding sexual health and to consider discussing contraceptive options to prevent unintended pregnancies and reduce the risk of STIs in the future.

In summary, while the timing of the bleeding may align with her menstrual cycle, the presence of pain and the sudden onset of bleeding warrant a careful evaluation to rule out any serious conditions. Encourage her to seek medical advice to ensure her health and well-being.
