Penile erection angle

Hello doctor, my penis has a slight downward curvature when erect.
When I exert force, the base can only remain at a 90-degree angle.
The more problematic issue is that if I try to bend the base upwards beyond 45 degrees, I cannot go any further upwards, and it causes slight pain at the base.
Previously, during intercourse with my girlfriend, I felt discomfort at the base upon insertion.
Could this be related to the curvature of my penis? Do I need to undergo surgery?

Penile curvature, often referred to as penile deformity, can be a source of concern for many men, particularly when it affects sexual function and comfort. The condition can manifest in various ways, including a downward bend during erection, which you have described. Understanding the causes, symptoms, and treatment options is essential for addressing this issue effectively.

Causes of Penile Curvature
1. Congenital Curvature: Some men are born with a curvature of the penis, which may not cause any issues until later in life. This is often referred to as congenital penile curvature.

2. Peyronie's Disease: This is a more common cause of acquired penile curvature. It occurs when fibrous scar tissue develops in the penis, leading to a bend during erection. The exact cause of Peyronie's disease is not fully understood, but it may result from trauma to the penis, genetic factors, or certain health conditions.

3. Trauma or Injury: Any injury to the penis, whether from sexual activity or other forms of trauma, can lead to curvature due to the formation of scar tissue.

4. Inflammatory Conditions: Conditions that cause inflammation in the penis can also lead to curvature. This includes infections or conditions like lichen sclerosus.

Symptoms
The primary symptom of penile curvature is the noticeable bend in the penis during an erection. Other symptoms may include:
- Pain during erections or sexual intercourse.

- Difficulty achieving or maintaining an erection.

- Changes in the shape or size of the penis over time.

In your case, the discomfort you experience when attempting to bend the penis upward beyond a certain angle suggests that there may be underlying scar tissue or other structural issues contributing to both the curvature and the pain.

Treatment Options
1. Observation: If the curvature is mild and not causing significant discomfort or problems with sexual function, a "watchful waiting" approach may be recommended.

2. Medications: In some cases, medications may be prescribed to help reduce pain or inflammation. However, there are no FDA-approved medications specifically for treating Peyronie's disease.

3. Penile Traction Therapy: This non-invasive treatment involves using a device to gently stretch the penis over time, which may help reduce curvature.

4. Injections: Some treatments involve injecting medications directly into the scar tissue to help break it down and reduce curvature.

5. Surgery: If the curvature is severe and causes significant pain or difficulties with sexual intercourse, surgical options may be considered. Surgical procedures can involve removing the scar tissue or placing a graft to straighten the penis. However, surgery carries risks and should be discussed thoroughly with a urologist.

When to Seek Help
Given your symptoms, particularly the pain associated with bending the penis and discomfort during intercourse, it is advisable to consult a urologist. They can perform a thorough examination, possibly including imaging studies, to determine the underlying cause of your curvature and recommend appropriate treatment options tailored to your specific situation.

In conclusion, penile curvature can be caused by various factors, and while it may not always require surgical intervention, it is essential to address any associated pain or functional issues. Seeking professional medical advice will provide you with the best course of action to manage your condition effectively.
