Enlarged testicles?
Hello Dr.
Hsu: Previously, my testicular epidermis was not very large, but recently it has swollen.
I'm not sure if it's due to my underwear being too tight or if it's because of the cold winter weather.
Also, I have noticed 3 to 4 hairs growing underneath my testicles; is this normal? Additionally, I would like to ask if it is possible to ejaculate semen if there is testicular cancer or another condition?
Xiao Ming, 10~19 year old female. Ask Date: 2016/01/25
Dr. Xu Weikai reply Urology
Xiao Ming: Hello.
1.
The swelling of the testicular epidermis needs to be assessed to determine if it is normal development or a tumor.
2.
It does not enlarge in winter.
3.
The testicles are located inside the scrotum, so they are not visible; how can you know if there is hair on the testicles? Note: If the scrotum is opened and hair is indeed found on the testicles, that is very serious.
4.
Ejaculation will still occur unless there is a very severe condition that completely damages the testicles.
If there are any unclear points, feel free to ask again for further discussion and research.
Online responses can only serve as a reference; if necessary, it is still important to seek medical attention at a healthcare facility.

Testicular enlargement can be a concerning symptom for many men, and understanding the potential causes and implications is crucial for addressing any underlying health issues. When a man notices an increase in the size of his testicles, it can be due to several factors, some benign and others more serious.
Firstly, it’s important to differentiate between true testicular enlargement and swelling of the surrounding tissues. True testicular enlargement may be caused by conditions such as epididymitis (inflammation of the epididymis), orchitis (inflammation of the testicle), or a hydrocele (fluid accumulation around the testicle). These conditions can result from infections, trauma, or other medical issues. In contrast, swelling that appears to be testicular enlargement could also be due to tight clothing or temperature changes, such as cold weather, which can cause the scrotum to contract and make the testicles appear larger.
Regarding the presence of hair beneath the testicles, it is completely normal for men to have hair in that area, as it is part of the natural development of male secondary sexual characteristics. The growth of hair is influenced by hormonal changes during puberty and can vary significantly from person to person.
Now, concerning your question about testicular cancer and its impact on ejaculation: Testicular cancer can present with a variety of symptoms, including a noticeable lump or swelling in the testicle, discomfort, or changes in the size or shape of the testicle. However, the presence of testicular cancer does not inherently prevent a man from ejaculating or producing semen. Many men with testicular cancer are still able to ejaculate, although treatment for the cancer, such as surgery or chemotherapy, can affect fertility and sexual function. If a man has concerns about his ability to ejaculate or experiences changes in his sexual function, it is essential to consult a healthcare provider for a thorough evaluation.
If you are experiencing testicular enlargement, it is advisable to seek medical attention. A healthcare professional can perform a physical examination and may recommend imaging studies, such as an ultrasound, to determine the cause of the enlargement. Early diagnosis and treatment are crucial, especially if there is a possibility of a serious condition like testicular cancer.
In summary, testicular enlargement can arise from various causes, including benign conditions and more serious health issues. Changes in hair growth are typically normal, and the ability to ejaculate is not necessarily affected by testicular cancer. If you have concerns about your symptoms, it is best to consult with a healthcare provider who can provide personalized advice and treatment options.
