Swelling at the posterior aspect of the second toe?

Hello Dr.
Chou! A year ago, while traveling, I wore higher sandals and experienced a sharp pain in my foot.
Initially, I thought it was due to a foreign object in my shoe and didn’t pay much attention.
However, after returning home, the discomfort intensified, prompting me to visit a large hospital for an X-ray and orthopedic evaluation.
The doctor diagnosed it as avascular necrosis and recommended surgery.
However, upon consulting another large hospital, they stated that there was no blackening at the back of my toe, indicating it was not avascular necrosis, and they only prescribed some topical ointment.
Later, I consulted a traditional Chinese medicine practitioner who mentioned a dislocation phenomenon (since then, I have been visiting the practitioner weekly to adjust the displaced bones in my foot, as they are very prone to dislocation).
They also indicated that this was due to insufficient muscle endurance in my foot and recommended exercises like the "golden rooster stands on one leg" to improve muscle endurance.
However, now whether I am walking or training for muscle endurance, I can easily hear the popping sound of bones dislocating and feel pain.
I would like to ask Dr.
Chou what could be causing the frequent dislocation of my bones, and comparing to my other foot, the tip of my second toe is still swollen.
Pressing or pulling the second toe causes significant pain, and if I accidentally kick something, there is even more noticeable swelling.
This issue has been troubling me for over a year, and I would greatly appreciate your guidance.
Thank you for taking the time to respond!

Dr. Zhou Jianwen reply Rehabilitation

Your issue may be due to ligament laxity and joint instability.
Please proceed to the rehabilitation department for further examination and treatment.

Understanding swelling and pain in the second toe can be quite complex, especially given your history of discomfort and the various diagnoses you've received. Based on your description, it seems that you have been experiencing persistent pain and swelling in your second toe for over a year, which has not only affected your daily activities but has also led to multiple consultations with different healthcare providers.

The initial pain you experienced after wearing high sandals could have been due to several factors, including improper footwear, which can lead to conditions like metatarsalgia or even a sprain. The fact that you felt a sharp pain suggests that there might have been an acute injury or strain to the soft tissues or bones in your foot. The subsequent diagnosis of potential bone necrosis is concerning, but the lack of blackening or other signs of necrosis on X-rays from other hospitals suggests that this may not be the case.

The mention of "displacement" by the traditional Chinese medicine practitioner indicates that there may be an issue with the alignment of the bones in your foot. This could be due to a variety of reasons, including ligament laxity, which can make the joints more prone to dislocation or subluxation. Ligament laxity can be hereditary or can develop due to overuse, injury, or even certain medical conditions that affect connective tissues.

The "clicking" sound you hear when moving your toe could be indicative of joint instability or a condition known as "crepitus," which is often associated with joint issues. This clicking can occur when the tendons or ligaments snap over the bones or when there is a misalignment in the joint. The swelling and pain you experience when pressure is applied or when you accidentally bump your toe could be a sign of inflammation in the joint or surrounding tissues, possibly due to repetitive strain or injury.

Given that your symptoms have persisted for over a year, it may be beneficial to seek a comprehensive evaluation from a foot and ankle specialist, preferably one who has experience with both orthopedic and podiatric conditions. They may recommend advanced imaging studies, such as an MRI, to assess the soft tissues, ligaments, and cartilage in your toe and foot. This can help rule out conditions like tendonitis, bursitis, or even early signs of arthritis.

In the meantime, there are several strategies you can implement to manage your symptoms:
1. Footwear: Ensure you are wearing supportive shoes that provide adequate cushioning and stability. Avoid high heels or sandals that do not support your arch.

2. Rest and Ice: Give your foot time to heal by resting it and applying ice to reduce swelling and pain.

3. Strengthening Exercises: Continue with the exercises recommended by your traditional Chinese medicine practitioner, as strengthening the muscles around the foot can help improve stability and reduce the risk of further displacement.

4. Physical Therapy: A physical therapist can provide tailored exercises to improve your foot's strength and flexibility, as well as techniques to manage pain.

5. Pain Management: Over-the-counter anti-inflammatory medications can help manage pain and swelling, but consult with your healthcare provider before starting any new medication.

6. Follow-Up: Regular follow-ups with your healthcare provider are essential to monitor your condition and adjust your treatment plan as necessary.

In conclusion, the combination of persistent pain, swelling, and the sensation of displacement in your second toe suggests that there may be underlying issues that need to be addressed. A thorough evaluation by a specialist can provide clarity and guide you toward effective treatment options. Remember, early intervention can often lead to better outcomes, so don’t hesitate to seek further medical advice.
