Premature ejaculation issues?

If masturbation often results in only a brief sensation of pleasure before ejaculation, does that indicate a discrepancy between pleasure and ejaculation? I typically ejaculate in about a minute, and sometimes even in less than a minute.
I experience pleasure around the frenulum and just below the penis, but the glans is only sensitive and does not provide pleasure.
I do not ejaculate from friction on the glans, although I have seen doctors mention that masturbation can lead to premature ejaculation.
Given my situation, I believe that intercourse would yield a similar outcome.
Aside from circumcision, medications for premature ejaculation, and anesthetics, what other methods are available? I am not very fond of premature ejaculation medications due to their side effects.
I would appreciate any guidance from a doctor.
Thank you.

Premature ejaculation (PE) is a common condition that affects many men, and it can be a source of significant distress and anxiety. Understanding the causes and exploring alternative solutions can be beneficial for those experiencing this issue.

Understanding Premature Ejaculation
Premature ejaculation is typically defined as ejaculation that occurs with minimal sexual stimulation, often within one minute of penetration, and it can happen before a man wishes it to. This condition can be classified into two types: lifelong (primary) PE, which has been present since the first sexual experiences, and acquired (secondary) PE, which develops after a period of normal sexual function.
The causes of PE can be multifactorial, including psychological factors such as anxiety, stress, or relationship issues, as well as physiological factors like hormonal imbalances, neurotransmitter levels, or even certain medical conditions. In your case, the rapid ejaculation during masturbation may indicate a sensitivity issue or a learned response that could carry over into sexual intercourse.

Alternative Solutions
While surgical options like circumcision or pharmacological treatments are often discussed, there are several alternative methods that may help manage premature ejaculation without the need for medication or surgery:
1. Behavioral Techniques:
- Start-Stop Technique: This involves stimulating the penis until you feel close to ejaculation, then stopping until the urge decreases. This can help you gain better control over your ejaculation.

- Squeeze Technique: Similar to the start-stop method, this involves squeezing the base of the penis when you feel close to ejaculation to reduce arousal.

2. Pelvic Floor Exercises:
- Strengthening the pelvic floor muscles through Kegel exercises can improve control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles that control urination.

3. Mindfulness and Relaxation Techniques:
- Practicing mindfulness can help reduce anxiety and improve sexual performance. Techniques such as deep breathing, meditation, or yoga can help you stay relaxed during sexual activity.

4. Desensitizing Products:
- Over-the-counter topical creams or sprays that contain numbing agents can reduce sensitivity in the penis, potentially delaying ejaculation. However, it’s important to use these products carefully to avoid affecting your partner’s sensation.

5. Counseling or Therapy:
- If psychological factors are contributing to your premature ejaculation, speaking with a therapist or counselor can help address underlying issues such as anxiety or relationship dynamics.

6. Communication with Your Partner:
- Open communication with your partner about your concerns can help alleviate anxiety and create a more supportive environment for both of you.

7. Lifestyle Changes:
- Regular exercise, a healthy diet, and avoiding excessive alcohol or recreational drugs can improve overall sexual health and performance.

Conclusion
While premature ejaculation can be frustrating, there are various strategies to explore beyond medications and surgical options. It’s essential to approach this condition holistically, considering both physical and psychological aspects. If you continue to experience difficulties, consulting with a healthcare professional specializing in sexual health can provide personalized guidance and support. Remember, you are not alone in this, and many men face similar challenges.
