Do I Need Circumcision? Pseudophimosis and Self-Confidence - Urology

Should I undergo circumcision in my situation?

Hello Dr.
Lee: I have done some research online beforehand, but I couldn't find anyone with a similar situation, so I am reaching out for clarification.
When my penis is not erect, I cannot see the glans at all, but I can manually retract the foreskin to fully expose it.
During an erection, the foreskin gradually retracts to about the position just behind the corona.
During sexual intercourse, I use my hand to pull the foreskin all the way back.
From the information I found online, it seems that most sources suggest that circumcision is not necessary.
During my military physical examination, there was no check for phimosis.
Japanese websites seem to refer to my condition as "pseudophimosis."
The issue is that when I went to a hot spring, I noticed that almost all the men there had their glans exposed even when not erect.
Even if it wasn't fully exposed, many had at least a third of it visible, and you could clearly see the urethral opening and the shape of the glans.
I was the only one with a significant amount of foreskin remaining, and the tip of my penis appeared triangular, which felt very odd.
It seemed like everyone was staring at my unusual shape.
A friend even joked that while other men had "mushrooms," I was the only one with "bullhorns" (referring to the red box of American corn snacks).
I always thought my condition was normal, but at the hot spring, I realized that I was among the very few with this situation, even though many men with noticeably smaller penises had their glans fully exposed.
This has made me feel quite self-conscious, and I am hesitant to participate in communal bathing situations.

Could you please advise whether I need to undergo circumcision for my condition?

It's written too complicated.
This is a matter of attitude.
If you're unsure, it's advisable to see a urologist for an examination to find out.

When it comes to the question of whether or not you need circumcision, particularly in the context of pseudophimosis, it’s essential to understand both the medical and psychological aspects involved. Based on your description, it seems that you have a condition where the glans (the head of the penis) is not visible when the penis is flaccid, but you can retract the foreskin to expose it fully. This is often referred to as "pseudophimosis," which is a condition where the foreskin can be retracted but may appear to be tight or not fully retractable when flaccid.

From a medical standpoint, pseudophimosis is generally not a cause for concern if you can retract the foreskin without pain and if there are no associated symptoms such as inflammation, infection, or difficulty urinating. Many men have variations in their anatomy, and what you are experiencing is not uncommon. The fact that you can fully retract the foreskin during an erection and during sexual activity indicates that there is no functional obstruction, which is a key factor in determining the necessity for circumcision.

Circumcision is typically recommended in cases of true phimosis, where the foreskin cannot be retracted at all, or in cases where there are recurrent infections or other complications. Since you have not reported any such issues, it is likely that circumcision is not necessary for you. However, if you have concerns about hygiene, recurrent infections, or if you experience pain or discomfort, it would be advisable to consult a urologist for a thorough examination and personalized advice.

On the psychological side, your feelings of self-consciousness and embarrassment in communal bathing situations are understandable. Body image issues can significantly affect self-esteem and confidence. It’s important to recognize that everyone has unique anatomical features, and variations are normal. The perception of what is "normal" can be skewed by societal standards and personal experiences, but it’s crucial to focus on your health rather than comparisons with others.

Building self-confidence in such situations can be challenging. Here are a few strategies that might help:
1. Education: Understanding that your condition is common and not a medical issue can alleviate some anxiety. Knowledge is empowering, and knowing that many men experience similar situations can help normalize your feelings.

2. Positive Affirmation: Practice positive self-talk. Remind yourself that your worth is not determined by your physical appearance. Focus on your strengths and qualities that make you unique.

3. Seek Support: If feelings of self-doubt and embarrassment persist, consider talking to a mental health professional. They can provide strategies to improve self-esteem and address body image concerns.

4. Gradual Exposure: If communal bathing situations make you uncomfortable, consider gradually exposing yourself to these environments. Start with smaller, less crowded settings where you feel safer, and gradually work your way up to larger groups.

5. Focus on Health: Prioritize your health and well-being over societal expectations. If you are healthy and can function normally, that is what truly matters.

In conclusion, based on your description, circumcision does not seem necessary for you at this time. However, if you have ongoing concerns, a consultation with a urologist would be beneficial. Additionally, addressing the psychological aspects of your self-image through education, positive affirmation, and possibly professional support can help you build confidence and reduce feelings of self-consciousness. Remember, everyone has their unique features, and embracing your individuality is key to self-acceptance.
