Are the antibiotics used to treat gastric ulcers ototoxic?
Hello, I currently have a gastric ulcer and need to undergo a 7-day triple therapy to treat Helicobacter pylori.
Today is the third day, but I started feeling a bit dizzy, and my left ear has some unusual sensations; when I speak, I hear a tingling sound, and the surrounding sounds seem different from my right ear.
I researched online and found that many antibiotics can be ototoxic, potentially causing hearing loss or deafness.
Could you please tell me if the following three medications I am taking have ototoxic effects? Could the issues with my left ear be related to these two antibiotics?
1.
AMOXICILLIN CAP 500MG (Amoxicillin Capsules 500 mg) - Take two capsules twice a day.
2.
KLARICID XK TAB 500MG (Clarithromycin Extended-Release Tablets 500 mg) - Take one tablet twice a day.
3.
PARIET EFC TAB 20MG (Rabeprazole Enteric-Coated Tablets 20 mg) - Take one tablet twice a day.
I am very concerned about the potential impact on my hearing, but I know I cannot stop the antibiotics on my own, as it may lead to antibiotic resistance.
The earliest appointment with a doctor at a large hospital is next week.
I would appreciate any clarification from a pharmacist.
Thank you!

Dr. Liao Lingmei reply Pharmacology
Hello, after reviewing the relevant information, it has been found that among the three medications, only KLARICID has reported side effects related to ear and inner ear labyrinth abnormalities (as mentioned in the attached medication leaflet).
This side effect is also described in pharmaceutical databases as reversible, meaning that it can resolve after discontinuation of the medication.
It is recommended to further consult with a pharmacist at a "major hospital," as they can assist in addressing your concerns and contacting your physician for further clarification.
Wishing you health and happiness!

The concern regarding the potential ototoxic effects of antibiotics, particularly in the context of treating stomach ulcers caused by Helicobacter pylori, is valid and deserves careful consideration. In your case, you are currently on a triple therapy regimen that includes Amoxicillin, Clarithromycin (Klaricid), and a proton pump inhibitor (Pariet).
To address your question directly: among the medications you are taking, Amoxicillin is generally not associated with ototoxicity. It is a commonly used antibiotic for various infections and is considered safe in terms of hearing. However, Clarithromycin, which is a macrolide antibiotic, has been reported in some cases to cause reversible auditory disturbances, including tinnitus and hearing changes. This is particularly noted in patients who may have pre-existing conditions or those who are on high doses or prolonged therapy.
The symptoms you are experiencing, such as dizziness and unusual sounds in your left ear, could potentially be linked to the Clarithromycin. It is important to note that these side effects are typically reversible upon discontinuation of the medication. However, stopping antibiotics prematurely can lead to treatment failure and the development of antibiotic resistance, which is a significant concern in the management of H. pylori infections.
Pariet (Rabeprazole), the proton pump inhibitor you are taking, is primarily used to reduce stomach acid and does not have known ototoxic effects. Therefore, it is unlikely to be the cause of your auditory symptoms.
Given your symptoms and the potential link to Clarithromycin, it is advisable to consult with your healthcare provider as soon as possible. They may recommend an evaluation by an ear, nose, and throat (ENT) specialist to rule out any other underlying issues that could be contributing to your symptoms, such as Eustachian tube dysfunction or middle ear problems.
In the meantime, it is crucial to monitor your symptoms closely. If you experience worsening dizziness, significant changes in hearing, or any other concerning symptoms, seek medical attention promptly. Your healthcare provider may consider adjusting your treatment regimen based on your symptoms and overall health status.
In summary, while Amoxicillin and Pariet are not associated with hearing loss, Clarithromycin may have a potential link to auditory disturbances. It is essential to discuss your symptoms with your doctor to ensure appropriate management of both your stomach ulcer and any auditory concerns. Remember, maintaining open communication with your healthcare team is vital for your health and well-being.
