Issues with the glans and foreskin?

1.
I have been experiencing small cracks in my foreskin since mid-March.
Sometimes after masturbation, it hurts, and there is even a little bit of blood (when covered with tissue, it shows a slight red stain).
Initially, I consulted a dermatologist and used the medication prescribed, but it was ineffective.
Later, I went to a urology department at a large hospital, where they diagnosed it as inflammation.
I took the prescribed medication and used topical treatments.
After refraining from masturbation for one to two weeks, the skin issue seemed to resolve, but the symptoms returned as soon as I masturbated again.
I then visited a well-known urology clinic nearby, where the doctor also diagnosed it as inflammation.
I was given a topical medication (a steroid ointment called "Xinghui Bijiang Ointment") and the doctor also loosened the ring at the tip of the foreskin, as I had a slight case of phimosis.
This allowed me to clean and engage in intercourse more easily.
However, after a week, the symptoms reappeared after masturbation.
I also had a blood sugar test (which was normal).
I occasionally spend money on that, but I practice safe sex by always using condoms.
The doctor prescribed another ointment for me, and I am almost finished with it now.
The symptoms seem to cause a bit of tearing after masturbation.
I suspect it might be because the scar hasn't fully healed and the pulling during masturbation aggravates it, especially since the opening at the tip of the foreskin is narrower.
I wonder if the doctor has encountered similar cases.
2.
Regarding the glans issue, when not erect, the foreskin retracts, and the glans appears whitish, with only the tip of the urethra being somewhat red.
There is a white area with a bit of redness extending backward at times.
Is this a normal phenomenon? I clean the glans daily during my shower to remove smegma.
Thank you, doctor.

Hello: It may be advisable to temporarily avoid ejaculation or sexual intercourse, but you should wash the area with clean water every morning and evening, and dry it (using a clean cotton swab).
Do not use soap or body wash.
Your situation is not uncommon; it involves skin injury leading to scarring, which then gets injured again, resulting in further scarring and skin changes.
Prolonged use of ointments can also cause the glans to appear unusual (this is common).
Please pay attention for the next 2-3 weeks, and I wish you good health.
(Maintain a light diet, minimize alcohol and spicy foods, and increase your water intake.)

Understanding glans and foreskin issues is crucial for maintaining genital health, especially when symptoms like cracks, pain, and inflammation arise. Based on your description, it seems you are experiencing a combination of symptoms that may be related to inflammation, irritation, or possibly an underlying condition such as phimosis or balanitis.

1. Symptoms and Initial Diagnosis: The small cracks in the foreskin, especially after sexual activity, suggest that there may be a mechanical irritation or a skin condition at play. The presence of blood after masturbation indicates that the skin is likely not healing properly, possibly due to repeated trauma. The fact that the symptoms improve when you abstain from sexual activity but recur upon resuming suggests that the area is sensitive and may not be fully healed.

2. Treatment Approaches: It’s good to hear that you have consulted multiple healthcare providers, including dermatologists and urologists. The use of topical steroids (like the cream you mentioned) can help reduce inflammation and promote healing, but they should be used cautiously and under medical supervision, as prolonged use can thin the skin and potentially worsen the problem.

3. Phimosis and Tight Foreskin: You mentioned that the doctor noted a tight foreskin. This condition, known as phimosis, can lead to difficulties in retracting the foreskin, which can cause irritation and inflammation. If the foreskin is too tight, it may not allow for proper cleaning, leading to a buildup of smegma, which can further irritate the area. In some cases, a minor surgical procedure called a circumcision or a preputioplasty may be recommended to alleviate the tightness.

4. Balanitis: The redness and irritation you observe on the glans, especially if it appears white in some areas, could indicate balanitis, an inflammation of the glans. This condition can be caused by infections (bacterial or fungal), irritants (like soaps or lotions), or even allergies. Maintaining proper hygiene is essential, but over-washing can also lead to irritation. It’s important to use mild, unscented soaps and to ensure that the area is thoroughly dried after washing.

5. Ongoing Symptoms: The recurring symptoms after sexual activity suggest that the area may still be sensitive or that there is an underlying issue that hasn't been fully addressed. It’s crucial to allow adequate time for healing. If the symptoms persist despite treatment, further evaluation may be necessary. This could include a referral to a specialist in sexual health or dermatology for a more in-depth assessment.

6. Preventive Measures: To prevent further irritation, consider the following:
- Avoid sexual activity until the area is fully healed.

- Use lubricants during sexual activity to reduce friction.

- Practice good hygiene by cleaning the area gently and avoiding harsh soaps.

- Monitor for any changes in symptoms, such as increased redness, swelling, or discharge, and report these to your healthcare provider.

7. Conclusion: It’s essential to communicate openly with your healthcare providers about your symptoms and treatment responses. If the current treatment plan isn’t effective, don’t hesitate to seek a second opinion or ask for alternative therapies. Your health and comfort are paramount, and persistent issues should be thoroughly investigated to ensure proper management and resolution.
In summary, while your symptoms may be common, they warrant careful attention and management to prevent complications and ensure a return to normal function.
