Injured urethra

Hello Doctor, a few months ago, I experienced frequent urination and discomfort in my lower abdomen after soaking in a hot spring.
I often hold my urine, and I'm wondering if this habit, combined with the hot spring visit, caused my symptoms.
I have been taking over-the-counter medication for three months, and it has now been four months, but the urethral opening is still slightly red, and I can see white spots inside the urethra.
Yesterday, due to anxiety, I went to see a doctor and explained my symptoms.
The doctor did not examine me physically but only listened to my concerns.
After taking the first dose of medication, I noticed that my urine was orange-red, which I had not experienced before.
Is this normal, or could it be related to the medication? The doctor mentioned that the white spots in the urethra need to be cleaned, but I am unsure whether rinsing the urethral opening with water would cause injury, or if I should use saline solution instead.
How should I clean it? Are the white spots possibly bacteria? Earlier this year, in February, I saw another doctor who used a plastic instrument to scrape inside the urethra for examination.
I am concerned that this scraping may have caused an injury.
I am unsure how to ask the doctor about my condition.
Thank you for considering my questions.

Urethral injury can be a complex issue, often resulting from various causes, including trauma, infections, or irritation from external factors. In your case, it seems that the symptoms began after visiting a hot spring, which may have contributed to your urinary discomfort. Frequent urination and discomfort in the lower abdomen can be indicative of a urinary tract infection (UTI) or irritation of the urethra, especially if you have a history of holding in urine for extended periods.

The symptoms you describe, such as the redness at the urethral opening and the presence of white spots inside the urethra, could suggest an infection or inflammation. The white spots may be due to a buildup of mucus, dead cells, or even bacterial colonization. It's essential to differentiate between these possibilities, as they will dictate the appropriate treatment.

The orange-red color of your urine after taking medication could be a side effect of certain medications, particularly those containing phenazopyridine, which is often used to relieve urinary pain. This medication can cause urine to appear orange or red, which is generally harmless but can be alarming if you're not expecting it. However, if you notice any other unusual symptoms, such as persistent pain, fever, or worsening of your condition, you should seek medical attention immediately.

Regarding the cleaning of the urethral area, it is crucial to approach this with caution. Rinsing the urethra with water or saline should be done gently to avoid causing further irritation or injury. It is not advisable to insert anything into the urethra without medical supervision, as this can lead to trauma or infection. If your doctor recommended cleaning the area, it would be best to clarify the method they suggest, as they may have specific instructions based on your condition.

If you feel that your concerns were not adequately addressed during your last visit, it is essential to communicate this to your healthcare provider. You have the right to ask questions and seek clarification about your symptoms and treatment options. If necessary, consider seeking a second opinion from another urologist or healthcare professional who specializes in urinary tract issues. They can provide a thorough examination, possibly including imaging studies or laboratory tests, to better understand the underlying cause of your symptoms.

In summary, urethral injuries can stem from various causes, and your symptoms warrant further investigation. It is crucial to maintain open communication with your healthcare provider, ensure you understand your treatment plan, and seek additional medical advice if your symptoms persist or worsen. Remember, your health and comfort are paramount, and you deserve comprehensive care and attention to your concerns.
