Menstrual changes after receiving the Lybrel injection?
Hello, doctor.
I would like to ask about my condition after undergoing surgery for endometriosis.
I received my first dose of short-acting leuprolide on August 2 and the second dose on September 5.
Recently, I have been experiencing a dull pain in my abdomen and have had diarrhea during my menstrual period (I had dysmenorrhea before the surgery).
On November 2, I noticed some bleeding, but it was dark brown, somewhat viscous, and not heavy.
Is this normal? Thank you for taking the time to answer my question.
Note: I want to get pregnant, so the doctor recommended administering two doses of leuprolide first.
MI, 30~39 year old female. Ask Date: 2017/11/03

Answer: After surgery for endometriosis, experiencing dark brown bleeding that is slightly viscous and low in volume is considered normal.

More Info
Leuprolide is a synthetic hormone that acts as a gonadotropin-releasing hormone (GnRH) agonist. It is commonly used in the treatment of conditions like endometriosis, uterine fibroids, and prostate cancer. After receiving leuprolide injections, many patients experience changes in their menstrual cycles due to the medication's effects on hormone levels.
In your case, it is not uncommon to experience abdominal discomfort and changes in menstrual bleeding patterns after receiving leuprolide. The medication works by suppressing the production of estrogen and progesterone, which can lead to a decrease in the thickness of the endometrial lining. This suppression can result in lighter periods or even amenorrhea (the absence of menstruation) in some patients. However, it can also cause irregular bleeding or spotting, especially in the initial months following the injections.
The symptoms you describe, such as abdominal pain and the presence of dark brown, thick discharge, can be attributed to the hormonal changes induced by leuprolide. Dark brown blood often indicates older blood that has taken longer to exit the uterus, which can be a normal occurrence during hormonal fluctuations. The fact that the bleeding is light and not accompanied by severe pain is generally a reassuring sign, but it is essential to monitor your symptoms closely.
The gastrointestinal symptoms, such as diarrhea during your menstrual period, can also be linked to hormonal changes. Hormones like prostaglandins, which are involved in regulating the menstrual cycle, can affect gut motility and lead to gastrointestinal discomfort. If you had a history of painful periods before your surgery, it is possible that the hormonal changes are exacerbating these symptoms.
Since you mentioned that you are trying to conceive and your doctor recommended the leuprolide injections, it is crucial to maintain open communication with your healthcare provider. They can provide personalized advice based on your medical history and current symptoms. If the abdominal pain becomes severe, if you experience heavy bleeding, or if you have any other concerning symptoms, it is essential to seek medical attention promptly.
In summary, the changes you are experiencing after the leuprolide injections can be considered normal, given the medication's effects on your hormonal balance. However, it is always best to discuss any concerns with your healthcare provider to ensure that your treatment plan aligns with your goals, especially regarding fertility. They may also suggest additional monitoring or alternative treatments if necessary.
