Regarding the coloration near the glans?
Hello Doctor, I have a problem with phimosis (but I can manually retract it to the coronal sulcus).
I wash the area thoroughly during showers, but recently I've noticed that the color around my glans is unusual.
Besides the urethral opening being red, the areas on both sides of the urethral opening appear grayish-white, and there are smooth white patches that do not hurt or itch.
Could this be what is referred to as leukoplakia, or is it just a fungal infection? Thank you for your response!

It is possible that what you mentioned can occur, and there may also be temporary color changes at times.
I recommend keeping the area clean and dry, but avoid excessive cleaning.
If possible, consult a urologist at a hospital for an examination (diagnosis cannot be made without an in-person evaluation).
Wishing you good health.

More Info
Understanding color changes around the glans can be concerning, and it is essential to approach this issue with care. Based on your description, it seems you are experiencing a change in color around the glans, specifically a grayish-white appearance, with the urethral opening being red. You also mentioned that the area feels smooth and does not cause pain or itching.
There are several potential causes for the color changes you are observing, and it is crucial to differentiate between them to determine the appropriate course of action. Here are some possible explanations:
1. Fungal Infection: A common cause of discoloration in the genital area is a fungal infection, such as candidiasis (yeast infection). This can lead to white patches or a grayish appearance, especially if there is an imbalance in the natural flora of the area. While you mentioned that there is no itching or pain, some fungal infections can be asymptomatic or present with mild symptoms.
2. Lichen Sclerosus: This is a chronic skin condition that can affect the genital area, leading to white patches and changes in skin texture. It often presents as smooth, white areas and can sometimes cause discomfort or itching, although not always. It is more common in men with phimosis or those who have had a history of skin conditions.
3. Balanitis: This is an inflammation of the glans, which can be caused by various factors, including poor hygiene, irritants, or infections. Balanitis can lead to redness and changes in color, and it may also present with swelling or discharge.
4. Pearly Penile Papules: These are small, benign growths that can appear around the glans and are often mistaken for other conditions. They are usually smooth, painless, and do not require treatment.
5. Vitiligo or Other Skin Conditions: Skin conditions like vitiligo can cause loss of pigmentation in certain areas, leading to white patches. This is less common in the genital area but is still a possibility.
Given the variety of potential causes, it is essential to consult a healthcare professional for an accurate diagnosis. A dermatologist or urologist can perform a physical examination and may recommend tests, such as a skin scraping or biopsy, to determine the underlying cause of the discoloration.
In the meantime, here are some general recommendations:
- Maintain Hygiene: Continue to practice good hygiene by gently washing the area with mild soap and water. Avoid harsh soaps or irritants that could exacerbate the condition.
- Avoid Irritants: Be cautious with personal care products, such as lotions or deodorants, that may come into contact with the genital area.
- Monitor Symptoms: Keep track of any changes in symptoms, such as the development of pain, itching, or discharge, and report these to your healthcare provider.
- Avoid Self-Diagnosis: While it may be tempting to self-diagnose based on online information, it is crucial to seek professional advice for an accurate assessment.
In conclusion, while the changes you are experiencing may be benign, they could also indicate an underlying condition that requires treatment. A healthcare professional can provide the necessary guidance and treatment options to address your concerns effectively.
