Meniscus Repair: Comparing Bone Marrow Cells and PRP for Recovery - Orthopedics

Meniscus repair

Hello Doctor: I underwent a meniscus arthroscopy repair surgery on April 13 and am currently in recovery.
Before the surgery, my doctor explained that there might be either a repair or removal of the meniscus, and that bone marrow cells would be extracted from my hip bone to be injected into the joint cavity of my meniscus to enhance its recovery.
However, this procedure would be out-of-pocket.
I initially thought it was the commonly used PRP (I had reviewed some information about PRP before the surgery), but the day before the surgery, the resident physician presented me with a consent form for an out-of-pocket procedure.
It was then that I learned that my treatment was not PRP, but rather bone marrow cells, which are more expensive than PRP, and that I would receive two injections.
The doctor mentioned that this type of bone marrow cell treatment would be superior to PRP and would have better regenerative capabilities, so I signed the consent.
I have been searching for information related to the use of bone marrow cells for meniscus treatment but have not found much.
I would like to ask the doctor: Is bone marrow cell therapy more advanced than PRP? Does it truly have better regenerative capabilities? Although I have already undergone the surgery, I still want to know the answer! It has been a week, but there is still swelling around the surgical area; is this normal? (I have been elevating my leg and applying ice consistently.) In the past few days, I have experienced a tearing, sharp pain at the surgical site; could this be due to a tear in the meniscus repair? Thank you.

There is limited information available regarding the efficacy of bone marrow cell injection therapy, and its effectiveness remains unclear.

Meniscus repair is a significant procedure for individuals suffering from knee injuries, particularly those involving the meniscus, which is a crucial cartilage structure in the knee joint. Your inquiry about the use of bone marrow cells versus Platelet-Rich Plasma (PRP) for recovery is quite relevant, especially given the advancements in regenerative medicine.

Bone marrow cells and PRP are both utilized in the field of orthopedics to enhance healing and recovery. However, they function differently and have distinct properties. Bone marrow cells, particularly mesenchymal stem cells (MSCs), are derived from the bone marrow and have the potential to differentiate into various cell types, including cartilage cells. This regenerative capability allows them to potentially repair damaged tissues more effectively than PRP, which primarily contains growth factors and platelets that promote healing but do not have the same regenerative potential as stem cells.

Research indicates that bone marrow-derived stem cells may provide better outcomes in terms of tissue regeneration and repair compared to PRP. Studies have shown that MSCs can not only aid in the repair of cartilage but also modulate inflammation and promote healing in a more comprehensive manner. However, it is essential to note that while bone marrow cells may offer advantages, the effectiveness can vary based on several factors, including the severity of the injury, the individual’s overall health, and the specific techniques used during the procedure.

Regarding your recovery, it is not uncommon to experience swelling and discomfort after meniscus repair surgery. Swelling is a natural response to surgery and can persist for several days to weeks as the body heals. The use of ice packs and elevating your leg are effective strategies to manage swelling. The sharp, tearing pain you describe could be related to the surgical site as it heals, but it is essential to monitor this closely. If the pain intensifies or if you notice any unusual symptoms, such as increased swelling, redness, or fever, it is crucial to contact your healthcare provider promptly.

In summary, while bone marrow cell therapy may offer superior regenerative capabilities compared to PRP, the choice of treatment should be tailored to the individual’s specific condition and needs. It is always advisable to discuss any concerns or questions with your orthopedic surgeon, who can provide personalized insights based on your surgical procedure and recovery progress. Your proactive approach to understanding your treatment options is commendable, and I encourage you to continue seeking information and support as you recover from your surgery.
