Phimosis, scrotal and groin swelling and pain?

I have a couple of questions:
1.
I had a circumcision when I was young, and now there is no foreskin covering my penis.
However, I have several hidden single and double fistulas on the left and right sides at the base of the penis, approximately 0.1 to 0.2 centimeters deep.
I usually pay close attention to hygiene, but recently I squeezed the area around the base of the penis, and a white substance resembling a pimple came out.
If I want to have that single or double fistula treated, should I make an appointment with a urologist at the hospital? How would the doctor go about removing the single and double fistulas?

For the past few years, I have occasionally experienced a dull, cramping pain in the area just below the pubic bone on the left side of the scrotum, which sometimes radiates to the testicles and can be quite uncomfortable, but it usually resolves on its own after a while.
