Post-Circumcision Care: Managing Pain and Urination Issues After Surgery - Urology

Six weeks after circumcision?

Hello, doctor.
I apologize for bothering you again.
It has been 34 days since my circumcision, and I am doing well with no major issues.
I have stopped applying medication for several days, and most of the stitches appear to have fallen out.
However, this morning during urination, I experienced some morning erections, and suddenly the urine stream was very forceful, causing a sharp pain at the tip of my penis.
When I urinated again at noon and wiped with toilet paper, I noticed a small amount of blood, but the quantity was very minimal; it was difficult to see without wiping.
Should I continue applying medication to aid in healing?
Additionally, after a month and a few days, I sometimes notice that my urine stream splits (usually during an erection) and sometimes it does not, but urination is very smooth when not erect.
Is this normal? Will it gradually improve over time? Lastly, should I intentionally allow friction, or should I wear briefs to minimize friction? I apologize for bothering you again, and I appreciate your guidance.
Wishing you good health and all the best.

Hello: The presence of bleeding suggests there may be a small wound.
Apply an ointment for protection.
After a circumcision, experiencing a split stream while urinating is normal and may occur more easily when straining; just avoid excessive force.
Currently, with a small wound, refrain from friction.
Wishing you good health.

Post-circumcision care is crucial for ensuring proper healing and minimizing discomfort after the procedure. It’s great to hear that you are 34 days post-surgery and that your recovery seems to be progressing well. However, it’s important to address the issues you are experiencing, such as pain during urination and the presence of slight bleeding.

Firstly, the strong urine stream you experienced, especially during morning erections, can indeed cause discomfort or pain at the surgical site. This is not uncommon, as the sensitivity of the area can be heightened post-surgery. The slight bleeding you noticed when wiping with tissue is also not unusual, particularly if there has been any irritation to the healing tissue. It’s essential to monitor this bleeding; if it increases or is accompanied by significant pain, you should consult your healthcare provider.

Regarding the use of topical medications, if your doctor previously advised you to apply ointments or creams, it may be beneficial to continue doing so, especially if you are experiencing any discomfort or irritation. These medications can help soothe the area and promote healing. However, if you have stopped using them and are not experiencing significant pain, it may be acceptable to continue without them. Always follow your healthcare provider's recommendations regarding medication use.

As for the issue of urine stream bifurcation, this can occur due to the anatomical changes following circumcision. It is generally considered normal for the urine stream to vary, especially during erections. Over time, as the area heals and the swelling decreases, you may notice improvements in the consistency of your urine stream. It’s important to remain patient during this healing process, as it can take several weeks for everything to normalize.

In terms of managing friction, wearing snug-fitting underwear, such as briefs, can help reduce unnecessary movement and friction against the surgical site. This can provide comfort and support during the healing phase. Avoiding excessive rubbing or irritation is key, so if you find that certain types of clothing cause discomfort, it may be wise to adjust what you wear.

Lastly, if you have any concerns about your healing process, such as persistent pain, unusual bleeding, or changes in urination patterns, it’s always best to reach out to your healthcare provider. They can provide personalized advice based on your specific situation and ensure that your recovery is on track.

In summary, continue to monitor your symptoms, maintain good hygiene, and consider using topical treatments if recommended. Wearing supportive underwear can help minimize discomfort, and be patient as your body heals. If any issues persist or worsen, do not hesitate to seek medical advice. Your health and comfort are paramount, and your healthcare provider is there to support you through your recovery.
