Persistent pain in the right upper arm?

I would like to ask the doctor, I am currently 18 years old, but I have experienced persistent pain in my right upper arm since childhood.
It is not particularly severe, but certain specific positions can trigger intense soreness, requiring me to change my posture to alleviate it somewhat.
This issue often occurs during sleep; for example, I cannot sleep on my left side, as it causes significant soreness in my right upper arm, preventing me from falling asleep.
Sometimes, I can relieve the pain by raising my arm, but as soon as I return to the original position, the soreness resumes.
I have researched many articles online, but since this has been an issue since childhood, I am unsure if it is caused by poor posture or activities like writing and sitting.
The primary symptom is soreness in the right upper arm.
I would like to know if there is a potential underlying cause for these symptoms or if there are ways to address them.

Hello: Based on your description, it may be necessary to consider cervical spine disorders.
Excessive force during cervical rotation and other external factors may lead to cervical instability, dislocation, and cervical disc herniation.
Typical discomfort symptoms associated with cervical radiculopathy include pain in the shoulder region or arm, as well as numbness, tingling, and weakness in the palm and fingers.

Treatment can be divided into medication management: When nerves are compressed, an inflammatory response occurs, leading to symptoms such as pain, numbness, and weakness.
Non-steroidal anti-inflammatory drugs (NSAIDs) can be used for treatment, as they have both anti-inflammatory and analgesic effects, which can reduce the inflammatory response and alleviate pain.
Physical therapy includes:
1.
Heat therapy: Primarily increases local blood circulation and improves muscle ischemia.
2.
Shortwave and ultrasound therapy: Mainly promotes blood circulation in deeper tissues.
3.
Transcutaneous electrical nerve stimulation (TENS) and medium-frequency vector interference waves: Can provide localized pain relief and alleviate tension and discomfort in the neck and shoulders.
4.
Cervical traction: Commonly used for degenerative cervical arthritis, disc herniation (cervical bone spurs), with associated nerve root compression, and cervical injuries without fractures or joint instability, where nerve compression is present.
Prior to traction, an X-ray examination should be performed to confirm joint stability.
Cervical collars: Used to prevent cervical instability and avoid exacerbating nerve tissue damage, suitable for injuries such as sports injuries, car accidents, or falls that result in excessive flexion or extension of the cervical spine.
I recommend that you consult with a neurologist or rehabilitation physician to discuss examination and treatment options.
Wishing you good health and peace of mind.

Chronic right upper arm pain, especially when it has persisted since childhood, can be attributed to a variety of causes, and understanding these can help in managing the condition effectively. Given your age and the nature of your symptoms, it is essential to consider both musculoskeletal and postural factors that may be contributing to your discomfort.

Possible Causes
1. Postural Issues: Poor posture, particularly during activities such as writing or sitting for extended periods, can lead to muscle imbalances and strain. If you tend to slouch or lean to one side, this could put undue stress on the muscles and tendons in your right arm, leading to chronic pain.

2. Muscle Strain: Overuse or repetitive strain injuries can occur, especially if you engage in activities that require repetitive arm movements. This can lead to conditions such as tendinitis, where the tendons become inflamed due to overuse.

3. Nerve Compression: Conditions such as thoracic outlet syndrome, where nerves or blood vessels are compressed in the space between your collarbone and first rib, can cause pain in the arm. This is often exacerbated by certain positions, such as sleeping on your side.

4. Rotator Cuff Issues: Although more common in older adults, issues with the rotator cuff can occur in younger individuals, especially if there is a history of repetitive overhead activities. A tear or inflammation in the rotator cuff can lead to pain that radiates down the arm.

5. Referred Pain: Sometimes, pain in the arm can be referred from other areas, such as the neck or shoulder. Conditions like cervical radiculopathy, where a nerve in the neck is compressed, can cause pain that radiates down the arm.

Management Strategies
1. Postural Correction: Focus on maintaining good posture while sitting and writing. Ergonomic adjustments, such as using a chair that supports your back and keeping your feet flat on the ground, can help reduce strain on your upper body.

2. Stretching and Strengthening Exercises: Incorporate exercises that strengthen the muscles around your shoulder and upper back. Stretching can also help alleviate tightness in the muscles that may be contributing to your pain. Consult with a physical therapist for a tailored exercise program.

3. Activity Modification: Pay attention to activities that exacerbate your pain. If certain positions or movements trigger discomfort, try to modify them. For example, if sleeping on your side causes pain, consider using a different sleeping position or adding pillows for support.

4. Heat and Cold Therapy: Applying heat can help relax tight muscles, while cold therapy can reduce inflammation. Alternating between the two may provide relief.

5. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage pain and inflammation. However, consult with a healthcare provider before starting any medication.

6. Professional Evaluation: If your symptoms persist or worsen, it may be beneficial to consult with a healthcare professional, such as a physical therapist or orthopedic specialist. They can perform a thorough evaluation, including physical examinations and possibly imaging studies, to determine the underlying cause of your pain.

7. Mindfulness and Relaxation Techniques: Stress can exacerbate pain perception. Techniques such as mindfulness meditation, yoga, or deep breathing exercises can help manage stress and improve your overall well-being.

Conclusion
Chronic right upper arm pain can stem from various causes, including postural issues, muscle strain, and nerve compression. By addressing these factors through proper posture, targeted exercises, and professional guidance, you can effectively manage your symptoms. If the pain continues to interfere with your daily activities or sleep, seeking a thorough evaluation from a healthcare provider is crucial to rule out any serious underlying conditions and to develop a comprehensive treatment plan tailored to your needs.
