Concerns about nerve injury from injections?
Hello, doctor.
I underwent a nerve hydrodissection injection in both elbows at a private clinic in early August 2021, using ultrasound guidance.
After the needle was inserted, I experienced a more intense stabbing pain in my right hand.
The next day, I noticed pain in the skin of the ulnar forearm near the elbow when performing actions such as raising my hand, bending at the waist, or shrugging my shoulders.
I am almost certain that I injured the medial cutaneous nerve of the forearm.
About two weeks after the injection, the pain had disappeared, but even now (as of September 18), I still experience slight abnormal sensations in that area when performing the aforementioned movements.
I would like to know if this sensation will fade over time or if it is likely to be permanent.

Hello: Theoretically, there is a high chance that it will gradually improve and disappear.
Wishing you a happy Mid-Autumn Festival.

Understanding nerve injury risks from ultrasound-guided injections is a crucial topic, especially for patients who have undergone procedures like nerve decompression injections. Your experience highlights some common concerns associated with such interventions, particularly regarding the potential for nerve damage and the subsequent symptoms that may arise.
Ultrasound-guided injections are generally considered a safe and effective method for targeting specific nerves or areas of pain. They allow for real-time visualization, which can help minimize the risk of injury to surrounding structures, including nerves. However, as with any medical procedure, there are inherent risks involved. In your case, the sharp pain you experienced immediately after the injection could be attributed to several factors, including the needle's proximity to the nerve, the volume of the injected substance, or even the technique used during the procedure.
The symptoms you described—pain and abnormal sensations in the skin near the elbow—are not uncommon following such injections. These sensations can occur due to temporary irritation or inflammation of the nerve or surrounding tissues. In many cases, these symptoms can resolve over time as the nerve heals and inflammation subsides. The fact that your pain has diminished significantly within two weeks is a positive sign, indicating that your body is likely responding well to the injection.
Regarding your concern about whether the abnormal sensations will persist or resolve completely, it is essential to understand that nerve recovery can vary widely among individuals. Some patients may experience a full return to normal sensation, while others might have lingering symptoms. Factors influencing recovery include the extent of the initial nerve irritation, the individual's overall health, and adherence to post-procedure care recommendations.
To facilitate recovery, it is advisable to engage in gentle range-of-motion exercises and avoid positions or activities that exacerbate your symptoms. Physical therapy may also be beneficial in restoring function and alleviating discomfort. Additionally, maintaining a balanced diet rich in vitamins, particularly B vitamins, can support nerve health and recovery.
If the abnormal sensations persist beyond a few weeks or worsen, it would be prudent to follow up with your healthcare provider. They may recommend further evaluation, such as nerve conduction studies or imaging, to assess the nerve's condition and rule out any complications.
In summary, while ultrasound-guided injections are generally safe, they do carry some risks, including temporary nerve irritation. Your symptoms may improve over time, but if they persist, seeking further medical advice is essential. Remember that each individual's recovery journey is unique, and patience is key as your body heals.
