Postoperative nerve scarring
Hello Dr.
Wang, I underwent a brain tumor resection surgery in June 2021, and postoperatively, I experienced facial numbness, trigeminal nerve issues, diplopia, and tinnitus.
I would like to know if ultrasound-guided injection could improve the symptoms of nerve scarring at the "central nervous system" level.
Thank you for your response.
Mr. Chen, 40~49 year old female. Ask Date: 2023/03/03
Dr. Wang Zhaomin reply Rehabilitation
Central nervous system lesions cannot be treated with ultrasound-guided injection therapy and can only address issues related to peripheral nerves and soft tissues of the muscles.

More Info
Ultrasound-guided injections are a valuable tool in the management of various pain conditions, particularly those involving peripheral nerves and soft tissues. However, when it comes to addressing issues related to central nervous system (CNS) injuries, such as nerve scarring following brain surgery, the effectiveness of ultrasound-guided injections is limited.
In your case, following a brain tumor resection, you are experiencing symptoms such as facial numbness, trigeminal nerve pain, diplopia (double vision), and tinnitus (ringing in the ears). These symptoms suggest that the nerve pathways in the central nervous system may have been affected by the surgery or the underlying condition. Unfortunately, ultrasound-guided injections are primarily effective for peripheral nerve issues and soft tissue problems, as they allow for precise delivery of medications (like corticosteroids or anesthetics) to areas where inflammation or nerve compression is present.
The central nervous system, which includes the brain and spinal cord, operates differently than peripheral nerves. Nerve scarring or damage in the CNS often leads to more complex issues that cannot be effectively treated with injections. Instead, the management of CNS-related symptoms typically involves a multidisciplinary approach, including medications for pain management, physical therapy, and possibly other interventions like neuromodulation techniques or rehabilitation therapies.
For your specific symptoms, it would be advisable to consult with a neurologist or a pain management specialist who has experience with post-surgical complications related to the CNS. They can provide a comprehensive evaluation and recommend appropriate treatments tailored to your condition. This may include medications such as anticonvulsants for nerve pain, antidepressants for chronic pain management, or even referral to a neurosurgeon if there are concerns about structural issues that may be contributing to your symptoms.
In summary, while ultrasound-guided injections can be beneficial for peripheral nerve issues, they are not suitable for treating nerve scarring or related symptoms in the central nervous system. A thorough evaluation by a specialist is essential to determine the best course of action for your post-surgical symptoms.
