Unidentified lesions on the foreskin?
Doctor, I have been experiencing this condition for three years.
The symptoms include unexplained sores under the glans, which sometimes resemble acne and at other times appear as round painful lesions.
I went to a major hospital for examination last year in April and May, and I underwent blood tests.
All sexually transmitted infections were ruled out, and I tested negative for hepatitis B and C.
The doctors often suggest that excessive sexual activity, masturbation, etc., could lead to skin irritation, but I had not engaged in any sexual activity for a year prior to the tests, and I also abstained for more than a month after the tests, yet the sores still appeared.
What could be causing this? There are no STIs, and I have not had sexual relations.
Each time, there are only 1 or 2 sores, and after applying medication for 14 to 20 days, the sores heal, but new ones appear within 5 days.
I can confirm that the sores develop as minor injuries within a day, and then become noticeable within 2 to 3 days.

Hello: Recurrent wounds may indicate diabetes.
It is advisable to consult a dermatologist, as persistent occurrences suggest an underlying issue that needs to be identified.
Wishing you good health.

More Info
Unexplained wounds under the foreskin can be a perplexing issue, especially when they persist over an extended period, as in your case. Given that you have already undergone thorough examinations, including blood tests for sexually transmitted infections (STIs) and hepatitis, and that these tests returned negative, it is essential to consider other potential causes and solutions.
Possible Causes
1. Friction or Irritation: Even if you have not engaged in sexual activity, friction from clothing, excessive masturbation, or even vigorous cleaning can lead to micro-tears in the sensitive skin of the foreskin. This can manifest as small wounds or sores that may become painful and inflamed.
2. Dermatological Conditions: Conditions such as lichen sclerosus, psoriasis, or eczema can cause skin changes and lesions in the genital area. These conditions may not be sexually transmitted but can lead to discomfort and skin breakdown.
3. Infections: While you have ruled out STIs, other infections, such as yeast infections or bacterial infections, can occur and may not always present with typical symptoms. These infections can lead to inflammation and ulceration of the skin.
4. Allergic Reactions: Allergies to soaps, detergents, or latex (from condoms) can cause localized irritation and wounds. It’s essential to consider any new products you may have introduced into your hygiene routine.
5. Underlying Health Issues: Conditions such as diabetes can affect skin healing and may predispose individuals to infections or skin breakdown. If you have any underlying health conditions, they may contribute to your symptoms.
Solutions and Recommendations
1. Gentle Hygiene: Ensure that you are cleaning the area gently with mild, unscented soap and water. Avoid harsh soaps or scrubs that can irritate the skin further.
2. Moisturizing: Applying a gentle, hypoallergenic moisturizer may help keep the skin hydrated and reduce the likelihood of cracks or wounds. Products containing aloe vera or vitamin E can be soothing.
3. Avoiding Irritants: Identify and avoid any potential irritants, including certain soaps, lotions, or laundry detergents. If you suspect an allergy, consider switching to hypoallergenic products.
4. Consult a Dermatologist: Since your primary care physician and urologist have not identified a clear cause, it may be beneficial to consult a dermatologist who specializes in skin conditions. They can perform a more detailed examination and may recommend a biopsy if necessary.
5. Topical Treatments: If you experience inflammation or pain, topical corticosteroids may be prescribed to reduce inflammation. However, this should only be done under the guidance of a healthcare professional.
6. Regular Monitoring: Keep a diary of when the wounds appear, any associated activities, and any products used. This information can help identify patterns or triggers that may be contributing to the issue.
7. Follow-Up Care: Since the wounds heal but recur, regular follow-up with your healthcare provider is crucial. They may consider further testing or alternative treatments based on your ongoing symptoms.
In conclusion, while unexplained wounds under the foreskin can be frustrating and concerning, understanding the potential causes and implementing appropriate care strategies can help manage the situation. It is essential to maintain open communication with your healthcare providers and seek further evaluation if symptoms persist or worsen.
