Regarding the small red spots on the foreskin?
Hello, Doctor Hsu.
I would like to inquire about some small red spots I noticed inside my foreskin while cleaning it during a shower.
They are not painful or itchy, and they don't feel raised.
I'm wondering if this could be a sexually transmitted infection? Will it resolve on its own? This is the first time I've experienced this, and I'm a bit anxious.
I have no sexual experience, and my foreskin is relatively long; when erect, I can only expose about one-third of the glans, but when not erect, it can be fully exposed.
Recently, while masturbating, I tried to fully expose the glans, but it felt tight, and the glans appeared dark red with a bit of purple.
Could this be the cause? Is there a need for circumcision? Thank you, Doctor!

Anonymous: Hello, it is difficult to determine whether it is a sexually transmitted infection (STI).
If it can heal on its own, it is likely a common infection.
It may be due to a tight foreskin, and you can gradually expose the glans to help stretch the foreskin.
If it is too tight, it can lead to poor blood circulation, and in severe cases, surgery is recommended.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

Understanding red spots under the foreskin can be a source of concern for many, especially when they notice changes in their genital area. In your case, the presence of small red spots on the inner side of the foreskin, without accompanying pain or itching, is often not indicative of a sexually transmitted infection (STI). However, it is essential to consider several factors to understand the situation better.
Firstly, the red spots you are observing could be a result of several benign conditions. One common cause is irritation or friction, especially if you have been more active in sexual activities or masturbation. The skin in the genital area is sensitive, and any increased friction can lead to temporary redness or spots. Additionally, if you have recently started to expose the glans (the head of the penis) more frequently, this could also lead to some irritation as the skin adjusts to the new exposure.
Another possibility is a condition known as balanitis, which is inflammation of the glans. This can occur due to poor hygiene, irritation from soaps or lotions, or even an allergic reaction. Balanitis often presents with redness, swelling, and sometimes discharge, but it can also manifest as red spots without significant discomfort.
Regarding your concern about whether these symptoms could be related to a sexually transmitted infection, it is important to note that STIs typically present with additional symptoms such as pain, itching, unusual discharge, or sores. Since you mentioned that you have no sexual experience, the likelihood of an STI is significantly reduced. However, if you have any doubts or if the symptoms persist or worsen, it is advisable to consult a healthcare professional for a thorough examination and appropriate testing.
As for the tightness you experience when attempting to fully retract the foreskin, this could indicate a condition known as phimosis, where the foreskin cannot be easily retracted over the glans. Phimosis is common in younger males and often resolves naturally over time. However, if it causes discomfort or hygiene issues, it may require intervention. In some cases, gentle stretching exercises or topical steroid creams can help. In more severe cases, circumcision may be recommended, but this is typically considered only if other treatments are ineffective.
In summary, the red spots you are observing are likely not a cause for alarm, especially given the absence of pain or itching. They could be due to irritation or a benign condition like balanitis. It is essential to maintain good hygiene and monitor the situation. If the symptoms persist, worsen, or if you develop new symptoms, seeking medical advice is the best course of action. A healthcare provider can offer a definitive diagnosis and discuss potential treatment options, including whether circumcision is necessary based on your specific circumstances.
