Neck muscle, bone, and nerve issues?

For the past two to three years, I have frequently experienced soreness from the left side of my neck, extending through the trapezius muscle to the scapula.
The inner side of my elbow also often feels sore.
When I raise and rotate my arm, I can hear a popping sound from the scapula.
While sleeping on my back, my left arm feels achy, and I find it more comfortable to sleep with my palm facing up.
Occasionally, I wake up with numbness in my hand, as if there is poor circulation.
When I turn my head to the left, I experience pain on that side.

Understanding neck and shoulder pain, especially when it has persisted for two to three years, can be quite complex. The symptoms you describe—pain radiating from the upper trapezius muscle down to the shoulder blade, discomfort in the inner elbow, and occasional numbness in the hand—suggest a combination of musculoskeletal issues and possibly nerve involvement.

Potential Causes
1. Muscle Strain and Tension: Chronic pain in the trapezius and surrounding muscles can often be attributed to muscle strain or tension. This is particularly common in individuals who have poor posture, spend long hours at a desk, or engage in repetitive motions. The trapezius muscle, which extends from the back of the neck to the shoulders, can become tight and painful due to prolonged static positions or stress.

2. Cervical Spine Issues: Given that you experience pain when turning your head to the left, there may be underlying issues with the cervical spine. Conditions such as cervical spondylosis (degenerative changes in the cervical spine) or herniated discs can lead to nerve compression, resulting in pain that radiates into the shoulder and arm. The fact that you experience numbness in your hand could indicate nerve involvement, possibly from a pinched nerve in the cervical region.

3. Rotator Cuff Problems: The sounds you hear when moving your shoulder may suggest issues with the rotator cuff, which is a group of muscles and tendons that stabilize the shoulder joint. Rotator cuff injuries can lead to pain and limited range of motion, especially when lifting or rotating the arm.

4. Thoracic Outlet Syndrome: This condition occurs when blood vessels or nerves are compressed in the space between the collarbone and the first rib. Symptoms can include pain in the neck and shoulders, numbness in the fingers, and weakness in the arm.

Solutions and Recommendations
1. Physical Therapy: Engaging in physical therapy can be highly beneficial. A physical therapist can design a personalized exercise program that focuses on strengthening the muscles around the neck and shoulders, improving posture, and increasing flexibility. Techniques such as manual therapy, ultrasound, and electrical stimulation may also be employed to alleviate pain.

2. Ergonomic Adjustments: If your work involves long hours at a desk, consider making ergonomic adjustments. Ensure your workstation is set up to promote good posture—your monitor should be at eye level, and your chair should support your lower back. Taking regular breaks to stretch and move can also help reduce muscle tension.

3. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and inflammation. However, it’s important to consult with a healthcare provider before starting any medication regimen.

4. Stretching and Strengthening Exercises: Incorporating stretching exercises for the neck and shoulders can help relieve tension. Strengthening exercises for the upper back and shoulder muscles can improve stability and reduce the risk of future injuries. Yoga and Pilates are also excellent for improving flexibility and core strength.

5. Consult a Specialist: If symptoms persist despite conservative treatment, it may be necessary to consult a specialist, such as an orthopedic surgeon or a neurologist. They may recommend imaging studies, such as an MRI or CT scan, to assess for any structural issues in the cervical spine or shoulder.

6. Lifestyle Modifications: Reducing stress through relaxation techniques, such as mindfulness or meditation, can also help alleviate muscle tension. Additionally, ensuring you have a supportive pillow and mattress can improve sleep quality and reduce morning stiffness.

Conclusion
In summary, your symptoms suggest a multifactorial issue involving muscle strain, potential cervical spine problems, and possibly nerve involvement. A comprehensive approach that includes physical therapy, ergonomic adjustments, and possibly medical intervention will likely yield the best results. It’s important to listen to your body and seek professional guidance to address these issues effectively.
