Ear pain when swallowing saliva?

Hello Doctor, a few days ago I discovered a small salivary gland cyst in my mouth.
It hurts when I swallow saliva.
After taking the medication prescribed by the ENT doctor, it has decreased in size, but I still experience pain when swallowing saliva, and my left ear also hurts when I swallow.
Could you please tell me what might be going on? Thank you, Doctor.

Hello, pain is often associated with inflammation.
Pain in the ear when swallowing is related to the same nerve supply.
Initial treatment can involve medication, but if symptoms persist without improvement, a follow-up visit for further medical evaluation and treatment is necessary.

Ear pain when swallowing can be a perplexing issue, often linked to various underlying conditions. The symptoms you describe—pain in the throat and ear during swallowing—suggest a possible connection between the throat and ear, which is anatomically supported by the Eustachian tube. This tube connects the middle ear to the back of the throat and helps equalize pressure in the ear. When you swallow, the muscles around the Eustachian tube contract, allowing air to enter the middle ear and equalize pressure. If there is any inflammation or blockage in this area, it can lead to discomfort or pain.

In your case, the presence of a salivary gland cyst in your mouth could be contributing to the pain. Salivary gland issues can lead to inflammation and discomfort, which may radiate to the ear, especially when swallowing. This is because the nerves that supply sensation to the throat and ear are closely related. The glossopharyngeal nerve, for instance, innervates both the throat and parts of the ear, which can explain why pain in one area can lead to discomfort in the other.

Additionally, if there is any inflammation in the throat—possibly due to the cyst or other factors like infection—this could further exacerbate the pain experienced during swallowing. Conditions such as tonsillitis, pharyngitis, or even referred pain from dental issues can also manifest as ear pain during swallowing.

To address your concerns, here are some potential causes and solutions:
1. Eustachian Tube Dysfunction: If the Eustachian tube is not functioning properly, it can lead to pressure imbalances in the ear, causing pain when swallowing. This can be evaluated by an ENT specialist through a tympanometry test or an audiogram.

2. Salivary Gland Issues: Since you mentioned a salivary gland cyst, it would be wise to follow up with your ENT or a specialist in oral and maxillofacial surgery. They can assess whether the cyst is causing significant issues and whether any treatment is necessary.

3. Infections: If there is an underlying infection in the throat or ear, antibiotics or other medications may be required. Your ENT can perform a thorough examination to rule out infections.

4. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may help alleviate pain during swallowing. Warm compresses applied to the ear may also provide some relief.

5. Hydration and Moisture: Staying well-hydrated can help keep the throat moist and may ease discomfort when swallowing. Additionally, using a humidifier in your home can help maintain moisture in the air, which can be soothing.

6. Avoid Irritants: If you smoke or are exposed to secondhand smoke, consider reducing exposure, as smoke can irritate the throat and exacerbate pain.

7. Follow-Up: If the pain persists or worsens, it is crucial to return to your healthcare provider for further evaluation. They may recommend imaging studies, such as an ultrasound or CT scan, to assess the salivary gland cyst and surrounding structures.

In conclusion, while ear pain during swallowing can stem from various causes, the relationship between the throat and ear is significant. Given your recent experience with a salivary gland cyst, it is essential to monitor your symptoms and consult with a healthcare professional for a thorough evaluation and appropriate management.
