Menstruation has not occurred for two months?

Hello Doctor: My last menstrual period was from January 14 to January 20, 2023.
I had intercourse on January 12 using a condom, and my period started two days later on January 14.
My menstrual cycle has been somewhat irregular in the past, but since August of last year, my periods have consistently started between the 10th and 14th of each month, continuing normally until January of this year.
However, after January, I have not had a period.
I took home pregnancy tests on February 14 and March 14, both of which showed a single line.
At the end of January, I experienced some anxiety and stress due to work, which has persisted until now, and I have also been going to bed very late since early February.
I have had a missed period once before, but it returned to normal the following month.
This is the first time I have missed two consecutive months.
I would like to ask the doctor if it is possible that I am pregnant, or if there could be other factors causing my missed periods.
Thank you for your patience in reading this.

Pingguo, 10~19 year old female. Ask Date: 2023/03/15

Apple: Hello, it is recommended to visit an obstetrician-gynecologist for a pregnancy test.
If the test shows no pregnancy, then an evaluation for menstrual induction can be considered.

More Info

Based on the information you've provided, it seems that you are experiencing a significant delay in your menstrual cycle, having missed your period for two months. You mentioned that your last menstrual period started on January 14, 2023, and you had sexual intercourse on January 12, using a condom. You also noted that you have been under stress and have had irregular sleep patterns, which can contribute to menstrual irregularities.

To address your question about the possibility of pregnancy: while it is theoretically possible to become pregnant even with condom use, the likelihood is relatively low if the condom was used correctly and did not break. The negative results from your home pregnancy tests on February 14 and March 14 further suggest that pregnancy is unlikely. Most home pregnancy tests are quite accurate when used correctly, especially after a missed period.

However, there are several other factors that could explain your missed periods. Stress is a well-known factor that can disrupt the hormonal balance necessary for regular menstruation. High levels of stress can lead to conditions such as hypothalamic amenorrhea, where the hypothalamus stops signaling the pituitary gland to produce hormones that regulate the menstrual cycle. Additionally, changes in sleep patterns, weight fluctuations, and underlying health conditions such as polycystic ovary syndrome (PCOS) can also lead to irregular or missed periods.

Given that you have experienced a similar situation in the past where your period was delayed but returned the following month, it is possible that your body is experiencing another temporary disruption. However, since this is the first time you have missed two consecutive periods, it would be advisable to consult with a healthcare provider. They can perform a thorough evaluation, which may include blood tests to check hormone levels, an ultrasound to assess your ovaries and uterus, and a review of your medical history and lifestyle factors.

In summary, while pregnancy is a possibility, the negative pregnancy tests and your history of stress and irregular sleep patterns suggest that other factors may be at play. It is essential to seek medical advice to rule out any underlying conditions and to discuss your symptoms in detail. Your healthcare provider can help you understand the reasons for your missed periods and guide you on the next steps to take for your reproductive health.
