Ankle, ankle sprain, ankle issues?
Hello, around the age of 13 or 14, I accidentally missed a step while going down the stairs and sprained my ankle.
The outer side of my ankle swelled up immediately, and I was unable to walk or put weight on it.
Later, I consulted a traditional Chinese medicine practitioner who only applied topical medication.
Since then, I have experienced recurrent pain and a feeling of swelling.
Last year, I had another injury from a fall, which affected the inner side of my ankle, resulting in bruising.
Even after the bruise faded, there is still a hard lump, and the pain continues to recur without improvement.
Both injuries are near the left ankle, and I experience intermittent pain every year.
Traditional Chinese medicine has not provided relief.
I would like to know what options are available to alleviate the pain, as it is so severe that the sensation in both feet feels completely different when I put weight on them.
Just stepping down is very painful.
Could you also recommend which medical specialty I should consult to address this issue? Currently, I am wearing an ankle brace for protection.
Thank you.

Dr. Wang Zhaomin reply Rehabilitation
When going out, you can wear ankle braces.
At home, you can apply heat or soak in warm water, and enhance stretching exercises along with strength training.
It is recommended to visit an orthopedic or rehabilitation specialist for an accurate diagnosis and appropriate treatment plan.

More Info
Chronic ankle pain, especially following injuries like sprains, can be a frustrating and debilitating condition. Your history of ankle injuries, including the initial sprain at age 13-14 and subsequent trauma from a fall, suggests that there may be underlying issues that have not been adequately addressed. The persistent pain and the sensation of hard lumps could indicate chronic inflammation, scar tissue formation, or even damage to the ligaments or cartilage in the ankle joint.
Understanding Chronic Ankle Pain
Chronic ankle pain can arise from several factors, including:
1. Ligament Damage: After a sprain, ligaments may not heal properly, leading to instability and recurrent pain.
2. Scar Tissue: Following an injury, scar tissue can form, which may restrict movement and cause pain.
3. Tendonitis: Overuse or improper healing can lead to inflammation of the tendons around the ankle.
4. Arthritis: Previous injuries can predispose the joint to early-onset arthritis, which can cause pain and stiffness.
5. Nerve Entrapment: Sometimes, nerves can become compressed or irritated due to swelling or scar tissue, leading to pain.
Rehabilitation and Treatment Options
Given your ongoing symptoms, a comprehensive rehabilitation approach is essential. Here are several effective strategies:
1. Physical Therapy: A physical therapist can design a personalized rehabilitation program focusing on strengthening the muscles around the ankle, improving flexibility, and restoring range of motion. They may use modalities such as ultrasound, electrical stimulation, or manual therapy to alleviate pain and promote healing.
2. Strengthening Exercises: Gradually incorporating exercises that strengthen the ankle and surrounding muscles can help stabilize the joint. This may include resistance band exercises, calf raises, and balance training.
3. Stretching: Regular stretching of the calf muscles and Achilles tendon can improve flexibility and reduce tension in the ankle.
4. Manual Therapy: Techniques such as joint mobilization or manipulation performed by a trained therapist can help improve joint function and reduce pain.
5. Orthotics: Custom orthotic devices or supportive footwear can help alleviate pressure on the ankle and improve alignment, which may reduce pain during activities.
6. Ice and Compression: Applying ice to the affected area can help reduce swelling and pain, especially after activities that exacerbate your symptoms. Compression wraps can also provide support and reduce swelling.
7. Medication: Over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help manage pain and inflammation. However, consult with a healthcare provider before starting any medication, especially if you have other health conditions.
8. Injections: In some cases, corticosteroid injections may be considered to reduce inflammation and pain, particularly if conservative measures do not provide relief.
9. Surgery: If conservative treatments fail and there is significant structural damage, surgical options may be explored. This could involve repairing damaged ligaments, removing scar tissue, or addressing any underlying joint issues.
When to Seek Further Help
If your symptoms persist despite these interventions, it may be beneficial to consult with an orthopedic specialist or a sports medicine physician. They can perform a thorough evaluation, including imaging studies like an MRI, to assess for any underlying issues that may require more targeted treatment.
Conclusion
Chronic ankle pain can significantly impact your quality of life, but with the right approach, it is possible to manage and reduce your symptoms effectively. A combination of physical therapy, strengthening exercises, and possibly medical interventions can help you regain function and alleviate pain. Remember to communicate openly with your healthcare providers about your symptoms and treatment preferences to find the best path forward.
