Anxiety disorder, fatigue, panic?

Hello, doctor.
I have experienced sudden fatigue and a feeling of weakness a few times at work.
Could this be related to anxiety disorder? I often feel tense, and sometimes I experience a sense of weakness afterward.
Additionally, I frequently wake up feeling tired, lacking energy, and feeling weak all over.
A few times, I have had panic attacks while out, and now I am hesitant to go outside because I feel very anxious.
Are there any methods or practices I can try to improve this? Also, I previously took Metoprolol and Escitalopram.
I took Metoprolol for six months, but I didn't notice much improvement, and I stopped Escitalopram due to severe nausea.
Are there any other medications I could consider trying?

Hello, doctor.
I have experienced sudden fatigue and a feeling of weakness at work several times.
Could this be related to anxiety disorder? A: It is possible.
After feeling very tense, sometimes there is a sensation of weakness, and often I wake up still feeling tired, lacking energy, and feeling weak all over.
A few times I have experienced sudden panic attacks while out, and now I am hesitant to go out because I feel very anxious.
Are there any methods I can practice or ways to improve this? A: You need to learn relaxation techniques, such as diaphragmatic breathing.
Additionally, I previously took medications like Mirtazapine and Sertraline.
I took Mirtazapine for six months but felt it was ineffective, and I had to stop Sertraline due to severe nausea.
I would like to ask the doctor if there are any other medications I could try.
A: There are many other medications available; please discuss this with your physician.

Hello, thank you for reaching out with your concerns regarding anxiety, fatigue, and panic attacks. Based on your description, it seems that you are experiencing symptoms that are commonly associated with anxiety disorders, particularly generalized anxiety disorder (GAD) or panic disorder. The feelings of sudden fatigue, a sense of weakness, and panic attacks can indeed be linked to anxiety.
Anxiety can manifest in various physical symptoms, including fatigue and a sense of being overwhelmed. When you experience anxiety, your body goes into a heightened state of alertness, which can lead to physical symptoms such as increased heart rate, muscle tension, and fatigue. This can create a cycle where anxiety leads to fatigue, and fatigue can, in turn, exacerbate feelings of anxiety. It's not uncommon for individuals with anxiety to feel exhausted even after a full night's sleep, as anxiety can disrupt sleep patterns and lead to poor sleep quality.

The panic attacks you mentioned, characterized by sudden feelings of intense fear or discomfort, can also contribute to the avoidance of certain situations, such as going outside. This is a common response among individuals who experience panic attacks, as they may begin to associate certain environments with the panic experience, leading to increased anxiety about those situations in the future.

To manage these symptoms, there are several strategies you can consider:
1. Cognitive Behavioral Therapy (CBT): This is a highly effective form of therapy for anxiety disorders. CBT helps you identify and challenge negative thought patterns and behaviors that contribute to anxiety. It can also teach you coping strategies to manage anxiety in real-time.

2. Mindfulness and Relaxation Techniques: Practices such as mindfulness meditation, deep breathing exercises, and progressive muscle relaxation can help reduce anxiety and improve your overall sense of well-being. These techniques can help ground you in the present moment and reduce the physiological symptoms of anxiety.

3. Regular Physical Activity: Engaging in regular exercise can significantly reduce anxiety levels. Physical activity releases endorphins, which are natural mood lifters, and can help improve your sleep quality.

4. Sleep Hygiene: Since you mentioned waking up feeling fatigued, it's essential to establish good sleep hygiene. This includes maintaining a consistent sleep schedule, creating a restful environment, and avoiding stimulants like caffeine close to bedtime.

5. Medication: Since you've previously tried medications like Metoprolol (美妥平) and Clonazepam (克憂果) with varying results, it may be worth discussing alternative medications with your healthcare provider. There are various classes of medications for anxiety, including selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), which may be more effective for you. Your doctor can help you find a medication that suits your needs and monitor any side effects.

6. Support Groups: Connecting with others who experience similar challenges can provide emotional support and practical coping strategies. Consider seeking out local or online support groups for individuals with anxiety disorders.

7. Professional Help: If you haven't already, consider consulting with a mental health professional who specializes in anxiety disorders. They can provide a comprehensive assessment and tailor a treatment plan that fits your specific needs.

It's important to remember that managing anxiety is often a multifaceted approach that may require time and patience. Don't hesitate to reach out for help, whether through therapy, support groups, or medication management. You are not alone in this, and with the right support and strategies, you can work towards feeling better and regaining control over your life.
