Peeling of the glans penis?

Hello doctor, about a month and a half ago, I noticed that the skin on my glans was peeling.
The following Saturday, I visited a urologist, who suggested that the peeling was likely caused by eczema and prescribed me a topical corticosteroid cream.
After using the cream for a week, the peeling subsided, so I stopped applying it.
However, about two weeks later, the peeling started again.
I then used the cream for another three to four days, and it seemed to resolve the peeling for about a week.
This cycle has happened twice, and now the peeling has returned.
Should I continue to apply the cream only when I have symptoms, or should I use it for an extended period? Or should I consider making another appointment? I read online that taking long showers can trigger eczema flare-ups, so I reduced my shower time from 30 minutes to 20 minutes, but I still have peeling symptoms.
Is this true? Are there other possible causes? Thank you.

Dr. Du Shixi reply Urology

Hello: It is recommended to use the ointment for an additional week.
Avoid showers, and limit the intake of spicy and irritating foods.
Do not use soap or body wash; make sure to dry off thoroughly after getting wet.
Wear loose-fitting underwear, and use protection during sexual activity.
This type of condition can recur multiple times a year, so it is essential to make lifestyle adjustments to prevent frequent relapses.
Wishing you good health.

More Info

Understanding and managing glans peeling can be a complex issue, as it may arise from various causes, including eczema, infections, or other dermatological conditions. Based on your description, it seems that you have been dealing with recurrent peeling of the glans for about six weeks, initially diagnosed as eczema by a urologist. You were prescribed a topical corticosteroid cream, which provided temporary relief but did not resolve the issue completely.

Causes of Glans Peeling
1. Eczema (Dermatitis): This is a common cause of skin peeling and can be triggered by various factors, including irritants, allergens, or even stress. In your case, the initial diagnosis of eczema seems plausible, especially if you have a history of sensitive skin or allergies.

2. Infections: Fungal infections, such as candidiasis, can cause peeling and irritation of the glans. Bacterial infections can also lead to similar symptoms. If there is any accompanying redness, swelling, or discharge, it might indicate an infection.

3. Irritation: Prolonged exposure to moisture, soaps, or other irritants can lead to skin peeling. You mentioned reducing your shower time, which is a good practice, as excessive moisture can exacerbate skin conditions.

4. Other Dermatological Conditions: Conditions such as psoriasis or lichen planus can also cause peeling of the skin in the genital area, although these are less common.

Treatment Options
1. Topical Corticosteroids: As prescribed, these can help reduce inflammation and itching associated with eczema. However, prolonged use should be monitored by a healthcare professional to avoid skin thinning or other side effects.

2. Moisturizers: Regular application of a gentle, fragrance-free moisturizer can help maintain skin hydration and barrier function, which may reduce peeling.

3. Antifungal or Antibacterial Treatments: If an infection is suspected, your doctor may prescribe antifungal or antibacterial medications.

4. Avoiding Irritants: Be mindful of soaps, lotions, or other products that may irritate the sensitive skin of the glans. Opt for mild, unscented products.

When to Seek Further Medical Attention
Given that your symptoms have recurred despite treatment, it would be advisable to return to your healthcare provider for a follow-up. They may consider:
- Re-evaluating the Diagnosis: A thorough examination may reveal other underlying conditions that were not initially considered.

- Conducting Tests: If an infection is suspected, swabs or cultures may be taken to identify the causative agent.

- Adjusting Treatment: Your doctor may recommend a different treatment regimen or additional therapies based on your current symptoms.

Conclusion
In summary, while it is understandable to want to manage your symptoms at home, the recurrence of peeling suggests that a more comprehensive approach may be necessary. Continuing to use the topical medication during flare-ups is reasonable, but it is essential to have a clear treatment plan and follow-up with a healthcare professional. They can provide guidance tailored to your specific situation and help you achieve long-term relief from your symptoms. Remember, maintaining good hygiene, avoiding irritants, and keeping the area moisturized are key components in managing skin conditions effectively.
