Emphysema
Hello, Doctor.
I would like to ask you about my father (81 years old) who has emphysema.
He experiences fever and sweating at night (sometimes during the day as well).
Blood tests show no signs of infection.
I am wondering if this condition is related to his emphysema.
Is it safe for him to receive a flu vaccine? I appreciate your help in answering my questions.
Thank you!

Dr. Xu Jialin reply Pulmonology
Patients with emphysema experience increased respiratory effort, which consumes a significant amount of calories and energy, leading to symptoms such as sweating or a sensation of body heat, although their actual body temperature does not indicate a fever.
The influenza vaccine provides appropriate protection for the elderly and patients with chronic pulmonary diseases, helping to prevent viral infections during epidemic periods that could lead to severe complications.
In principle, as long as there is no current fever or infection and the patient has no allergies to the vaccine, vaccination is recommended.

More Info
Emphysema is a chronic lung condition that falls under the umbrella of chronic obstructive pulmonary disease (COPD). It primarily affects the air sacs in the lungs, leading to difficulty in breathing and reduced oxygen exchange. The symptoms of emphysema can vary widely among individuals, and it is not uncommon for patients to experience night sweats or increased sweating during the day.
In your father's case, the night sweats he is experiencing may not be directly caused by emphysema itself. While emphysema can lead to respiratory distress and other complications, night sweats are often associated with a variety of other factors, including infections, hormonal changes, or even certain medications. Since you mentioned that blood tests have ruled out infections, it may be worthwhile to consider other potential causes. For instance, conditions such as anxiety, hyperhidrosis (excessive sweating), or even sleep apnea could contribute to his symptoms.
It is also important to note that older adults, particularly those with chronic health conditions, may experience changes in their thermoregulation, which can lead to increased sweating at night. Additionally, certain medications used to manage emphysema or other coexisting conditions could also have side effects that include sweating. Therefore, it would be prudent to review his current medications with his healthcare provider to determine if they could be contributing to his symptoms.
Regarding vaccination, particularly for the flu or pneumonia, it is generally recommended for individuals with chronic lung conditions like emphysema. Vaccinations can help prevent respiratory infections that could exacerbate his condition. However, it is essential to consult with his healthcare provider before proceeding with any vaccinations. They can assess his overall health status, including his lung function and any other underlying conditions, to determine the appropriateness of vaccination at this time.
In summary, while night sweats can be concerning, they may not be directly linked to emphysema. It is crucial to explore other potential causes and discuss them with a healthcare professional. Vaccination against respiratory infections is typically advisable for patients with emphysema, but a thorough evaluation by a physician is necessary to ensure safety and efficacy. Regular follow-ups and open communication with his healthcare team will be vital in managing his condition effectively.
