Urinalysis
Hello: I did not eat anything after midnight the night before my health check-up.
I urinated at 7 AM the next morning and did not eat again until I urinated at 10:30 AM, which was used for the urine test.
I would like to ask if the urine collected after fasting for ten and a half hours could slightly affect the accuracy of the results.
Otherwise, I really cannot understand how my triglyceride levels, despite my usual balanced diet and somewhat plump physique, could be tested as too low, leading to recommendations for better nutritional balance.
How could this happen?

Dear Dr.
Lin Ming-Hsing, Director of Family Medicine at Tainan Hospital, Department of Health:
I have a point of clarification: triglycerides are not detected through urine tests but rather through venous blood sampling.
Although the concentration of triglycerides in the blood is significantly related to dietary intake, it is also closely associated with any deficiencies or disorders in the body's enzymatic metabolism.
A vegetarian diet that is not high in fats can still lead to hyperlipidemia.
Therefore, I suggest that you need not worry about this matter.
As you mentioned, eating a variety of foods without being picky is perfectly fine.
Wishing you good health and all the best from Tainan Hospital!

More Info
Fasting before a urine test can indeed have an impact on the accuracy of certain results, but it largely depends on the specific tests being conducted and the individual's overall health and dietary habits. In your case, fasting for about ten and a half hours before providing a urine sample is generally acceptable for most routine urine tests. However, it is essential to understand how fasting can influence various components of urine analysis.
1. Concentration of Urine: Fasting can lead to dehydration, which may concentrate the urine. This concentration can affect the levels of certain substances in the urine, such as electrolytes, proteins, and glucose. If your urine is more concentrated than usual, it could lead to misleading results, such as elevated levels of certain metabolites or waste products.
2. Ketones: If you fast for an extended period, your body may start to break down fat for energy, leading to the production of ketones. The presence of ketones in urine can indicate that your body is in a state of ketosis, which is not typical for someone who has been eating a balanced diet. This could potentially confuse the interpretation of your urine test results.
3. Creatinine Levels: Fasting can also affect creatinine levels in the urine. Creatinine is a waste product from muscle metabolism, and its levels can vary based on hydration status and dietary intake. If you were dehydrated due to fasting, your creatinine levels might be higher than expected.
4. Impact on Lipid Levels: Regarding your concern about low triglyceride levels, fasting can influence lipid metabolism. Typically, triglyceride levels are measured after fasting for 9-12 hours. If you fasted and then had a urine test, it might not directly affect triglyceride levels in urine, but it could reflect changes in your overall metabolism. If your diet is generally balanced and you have no underlying health issues, low triglyceride levels might warrant further investigation to rule out metabolic disorders.
5. Nutritional Balance: If you are advised to maintain a balanced diet, it is crucial to consider that fasting can lead to temporary fluctuations in various metabolic markers. If you are consistently eating a balanced diet but still see abnormal results, it may be worth discussing with your healthcare provider. They might suggest further testing or a review of your dietary habits to ensure there are no underlying issues affecting your health.
In summary, while fasting for a urine test is generally acceptable, it can influence the concentration and composition of your urine, potentially affecting the accuracy of certain results. If you have concerns about your test results or dietary habits, it is advisable to consult with your healthcare provider for a comprehensive evaluation and personalized recommendations. They can help clarify any discrepancies and guide you on maintaining optimal health through proper nutrition and lifestyle choices.
