Lumbar issues?

Hello, doctor.
I have pain in the left side of my lower back.
Symptoms: Initially, I felt soreness in my lower back upon waking up in the morning.
After a few days, there was a day when I woke up with significant pain in my lower back, making it impossible to lie flat and return to sleep.
However, the pain subsided after the morning.
I sleep on a wooden bed! Recently, the pain in my left lower back has returned; although it is not very intense, it is persistent.
I have already gone to the hospital for an examination, but the report has not yet come out.
The physician did not provide much explanation, only saying to wait for the report.
Based on my symptoms, could you please tell me what possible causes there might be? Thank you.

Dr. Zeng Guosen reply Internal Medicine

Hello: Lower back pain, especially in the morning.
Based on the limited symptom description from online users, it is not possible to provide a definitive diagnosis.
Myofascial inflammation or ankylosing spondylitis should be evaluated first, followed by assessment of visceral and retroperitoneal diseases, and only then consider issues related to the mattress.
It is recommended to consult with an orthopedic specialist or a rheumatologist for further examination.
Sincerely, Dr.
Tseng Kuo-Sen, Taoyuan General Hospital.

Hello! Thank you for sharing your symptoms regarding your left lower back pain. Based on your description, it seems that you are experiencing discomfort primarily in the left side of your lower back, which has been persistent and has worsened after certain sleeping positions. Here are some potential causes and insights into your condition.

1. Muscle Strain or Sprain: One of the most common causes of lower back pain is muscle strain or sprain. This can occur due to improper lifting, sudden movements, or even sleeping in an awkward position. Given that you mentioned waking up with soreness and then experiencing significant pain that prevents you from lying flat, it is possible that you may have strained a muscle in your lower back.

2. Poor Sleeping Posture: You mentioned sleeping on a wooden board bed. While a firm mattress can be beneficial for some, it may not provide adequate support for everyone. Sleeping on a very hard surface can lead to misalignment of the spine, especially if you are not using supportive pillows. This misalignment can contribute to muscle tension and pain.

3. Herniated Disc: Another possibility is a herniated disc, particularly if you have had any history of heavy lifting or sudden movements. A herniated disc can cause localized pain and may also lead to nerve irritation, which can cause pain to radiate down the leg. However, this typically presents with additional symptoms such as numbness or tingling in the legs.

4. Sciatica: If the pain radiates down your leg or is accompanied by tingling or numbness, it could be related to sciatica, which is caused by irritation of the sciatic nerve. This can occur due to a herniated disc or other spinal issues.

5. Kidney Issues: Although less common, pain in the lower back can sometimes be related to kidney problems, such as a kidney stone or infection. This is usually accompanied by other symptoms, such as changes in urination or fever.

6. Degenerative Disc Disease: As we age, the discs in our spine can degenerate, leading to pain and discomfort. This condition is often characterized by chronic pain that can be exacerbated by certain movements or positions.

7. Sacroiliac Joint Dysfunction: The sacroiliac joint connects the spine to the pelvis and can become inflamed or dysfunctional, leading to pain in the lower back, particularly on one side.

Given that you have already sought medical attention and are awaiting your test results, it is essential to follow up with your healthcare provider once the reports are available. They will be able to provide a more accurate diagnosis based on the findings.

In the meantime, here are some general recommendations to help manage your pain:
- Rest: Avoid activities that exacerbate your pain, especially heavy lifting or prolonged sitting.

- Heat Therapy: Applying heat to the affected area can help relax tight muscles and alleviate pain.

- Gentle Stretching: Engage in gentle stretching exercises to improve flexibility and reduce tension in the back muscles.

- Posture Awareness: Pay attention to your posture while sitting and sleeping. Consider using a supportive mattress and pillow.

- Physical Therapy: If your pain persists, a physical therapist can provide tailored exercises and treatments to help strengthen your back and improve mobility.

Remember, it is crucial to consult with your healthcare provider for a proper diagnosis and treatment plan tailored to your specific condition. I hope you find relief soon!
