Fish oil supplementation nutritional issues?
Hello, Nutritionist: It is known that EPA has anticoagulant effects.
If the fish oil contains 250mg of DHA and 36mg of EPA per capsule (approximately a 7:1 ratio), and the product is specifically labeled for pregnant women, can this product be safely consumed until delivery and during breastfeeding? I would greatly appreciate your response to this question.
Thank you very much!
Jerry, 30~39 year old female. Ask Date: 2010/03/07
Dr. Shi Guimei reply Nutrition
Dear Jerry,
Hello! During pregnancy, it is best to consume a variety of foods in a balanced manner.
Aim for 2 cups (240 cc each) of dairy products daily.
If you wish to take supplements, a multivitamin is sufficient; I do not recommend additional products such as EPA or DHA.
Wishing you safety, health, and happiness!
Sincerely,
Shih Kuei-Mei, Director of the Nutrition Department, Taoyuan Hospital, Ministry of Health and Welfare

When it comes to the safety of fish oil supplements during pregnancy, there are several important factors to consider. Fish oil is rich in omega-3 fatty acids, specifically EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), both of which are crucial for fetal development, particularly for brain and eye health. However, the safety of these supplements, especially in relation to their EPA content, has raised concerns among healthcare professionals.
In your case, you mentioned a fish oil supplement with a composition of DHA 250mg and EPA 36mg, which gives a ratio of approximately 7:1. This is a relatively favorable ratio, as higher levels of EPA have been associated with potential risks during pregnancy, particularly concerning blood coagulation. EPA has anticoagulant properties, which means it can affect blood clotting. This is particularly relevant during labor, where excessive bleeding can be a concern.
The general consensus among healthcare providers is that while DHA is beneficial and important for fetal development, the intake of EPA should be monitored, especially in the later stages of pregnancy. The American Pregnancy Association suggests that pregnant women can safely consume fish oil supplements, but they should be cautious about the EPA content. Supplements specifically labeled for pregnant women typically have a balanced ratio of DHA to EPA, which minimizes potential risks.
Given that your supplement is labeled for pregnant women, it likely adheres to safety standards set for prenatal nutrition. However, it is still essential to consult with your healthcare provider before continuing or starting any supplement, especially as you approach delivery. They can provide personalized advice based on your health history and any specific concerns you may have.
In terms of dosage, the typical recommendation for omega-3 fatty acids during pregnancy is around 200-300mg of DHA per day. Since your supplement provides 250mg of DHA per capsule, taking one capsule daily would be appropriate. However, always follow your healthcare provider's recommendations regarding dosage and duration of use.
Additionally, it is worth noting that obtaining omega-3 fatty acids from whole food sources, such as fatty fish (like salmon, sardines, and mackerel), is often encouraged over supplements. These foods not only provide DHA and EPA but also come with other beneficial nutrients and are less likely to pose risks associated with high concentrations of isolated compounds.
In summary, based on the information provided, it appears that your fish oil supplement can be safely taken until delivery, provided it is specifically formulated for pregnant women and you are following the recommended dosage. However, always prioritize discussions with your healthcare provider to ensure that your supplementation aligns with your individual health needs and the health of your baby. Regular prenatal check-ups will also help monitor your health and the development of your baby, allowing for any necessary adjustments to your diet or supplementation.
