Post-sinus surgery
Hello, Doctor.
I would like to inquire about some post-operative issues regarding my sinusitis.
About a month ago, I underwent surgery for chronic sinusitis with nasal polyp growth.
However, since the surgery, I have been experiencing persistent nasal congestion, which worsens during sleep.
A few days ago, during my follow-up appointment, the doctor performed an endoscopy and mentioned that a few granulation tissues have developed, and my nasal secretions are increased.
I understand that there is a possibility of recurrence, but a month seems excessive.
Additionally, if the congestion was due to polyps, why am I still experiencing nasal blockage? I hope you can provide some clarification.
Thank you.

Dear Ah-Ling,
Sinusitis is a chronic condition that requires ongoing medication treatment even after surgery.
There are many causes of nasal congestion; sometimes it is due to excessive secretions causing obstruction, not just a problem with nasal polyps.
It is recommended to continue follow-up visits and discuss with your surgeon.
Wishing you peace and health,
Dr.

Post-sinus surgery complications, particularly after procedures for chronic sinusitis and nasal polyps, can be quite complex and frustrating for patients. It is not uncommon for individuals to experience persistent nasal congestion even after surgical intervention. Here, I will address your concerns regarding ongoing nasal congestion, the formation of granulation tissue, and the potential for recurrence of symptoms.
Firstly, it is essential to understand that sinus surgery, particularly endoscopic sinus surgery, aims to improve drainage and ventilation of the sinuses. However, the healing process can vary significantly from person to person. After surgery, it is normal for some swelling and inflammation to occur as the tissues heal. This can lead to temporary nasal congestion, which may persist for several weeks or even months post-operatively.
The presence of granulation tissue, as noted during your follow-up endoscopy, is a common occurrence after sinus surgery. Granulation tissue is a type of healing tissue that forms as part of the body’s natural response to injury. While it is a normal part of the healing process, excessive granulation tissue can contribute to nasal obstruction and may require further intervention, such as cauterization or additional surgical procedures, to remove it.
Regarding your concern about nasal polyps and the possibility of recurrence, it is important to note that chronic sinusitis and nasal polyps are conditions that can have a tendency to recur. Even after surgical removal, there is a chance that new polyps can develop, especially if the underlying causes of inflammation, such as allergies or environmental factors, are not addressed. This is why ongoing management, including the use of nasal corticosteroids, saline irrigation, and possibly allergy treatments, is often recommended after surgery to help reduce the risk of recurrence.
Additionally, the increased nasal secretions you are experiencing can be attributed to several factors, including post-operative inflammation, infection, or even allergic reactions. It is crucial to monitor the characteristics of your nasal discharge; if it becomes purulent (thick and colored), this may indicate an infection that requires medical attention.
In terms of your sleep disturbances due to nasal congestion, it is essential to explore options for alleviating symptoms. This may include the use of nasal saline sprays, decongestants (if appropriate), or other medications prescribed by your healthcare provider. Additionally, sleeping with your head elevated can help improve nasal drainage and reduce congestion during the night.
In conclusion, while persistent nasal congestion after sinus surgery can be concerning, it is not uncommon. The formation of granulation tissue and the potential for recurrence of nasal polyps are factors that can contribute to ongoing symptoms. It is essential to maintain open communication with your ENT specialist, who can provide tailored advice and treatment options based on your specific situation. Regular follow-up appointments are crucial to monitor your recovery and address any complications that may arise. If symptoms persist or worsen, further evaluation and intervention may be necessary to ensure optimal healing and quality of life.
