Swelling and pain in the ankle?

Hello doctor, three days ago in the afternoon (around 2 PM) at home, I suddenly felt a bit of soreness in the tendons behind my ankle extending to my calf (I didn't think much of it).
After sitting and using the computer for about half an hour, I noticed some swelling around my right ankle (it was a bit painful at that time).
I even lightly massaged the swollen area with my hand.
By the evening, my entire ankle was painful.
The next day, both the inner and outer sides of my ankle were swollen.
If I slightly flex my foot forward, the tendons in the back of my ankle hurt, and if I flex it upward, the tendons also hurt.
However, there is pain when I put weight on the area around my ankle.
What could be the cause of this?

Dr. Shi Guozheng reply Orthopedics

It is likely tendonitis, and treatment with medication may help alleviate the symptoms.
It would be advisable to see a physician for a proper evaluation.

Swollen and painful ankles can be caused by a variety of factors, and understanding the underlying reasons is crucial for effective management and treatment. Based on your description, it seems that you are experiencing discomfort and swelling around your ankle, particularly after sitting for a period of time. Here are some possible causes and solutions to consider.

Possible Causes:
1. Injury or Strain: It’s possible that you may have sustained a minor injury or strain to the ligaments or tendons around your ankle. This can happen even without a significant incident, especially if you have been engaging in activities that put stress on your legs.

2. Poor Circulation: Sitting for extended periods can lead to poor circulation in the legs, which may cause swelling. When blood flow is restricted, fluid can accumulate in the lower extremities, leading to swelling and discomfort.

3. Tendonitis: Inflammation of the tendons, known as tendonitis, can cause pain and swelling in the ankle area. This condition is often exacerbated by repetitive movements or overuse.

4. Bursitis: The bursae are small fluid-filled sacs that cushion the bones and tendons around joints. Inflammation of these sacs can lead to swelling and pain, particularly in the ankle.

5. Arthritis: Conditions such as osteoarthritis or rheumatoid arthritis can lead to joint swelling and pain. If you have a history of joint issues, this could be a contributing factor.

6. Gout: Gout is a type of arthritis characterized by sudden and severe pain, redness, and swelling in the joints, often affecting the big toe but can also impact the ankle.

7. Infection: Infections in the ankle joint or surrounding tissues can cause swelling, redness, and pain. This is often accompanied by fever and other systemic symptoms.

8. Fluid Retention: Conditions such as heart failure, kidney disease, or liver problems can lead to fluid retention, resulting in swollen ankles.

Solutions and Recommendations:
1. Rest and Elevation: Avoid putting weight on the affected ankle and elevate it above the level of your heart to reduce swelling.

2. Ice Therapy: Applying ice to the swollen area for 15-20 minutes every few hours can help reduce inflammation and numb the pain.

3. Compression: Using an elastic bandage or compression sock can help manage swelling and provide support to the ankle.

4. Over-the-Counter Pain Relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help alleviate pain and reduce inflammation.

5. Gentle Stretching and Strengthening Exercises: Once the acute pain subsides, gentle stretching and strengthening exercises can help restore mobility and prevent future injuries.

6. Hydration and Diet: Ensure you are well-hydrated and consider reducing salt intake to help manage fluid retention.

7. Consult a Healthcare Professional: If the swelling and pain persist or worsen, it is important to consult a healthcare professional for a thorough evaluation. They may recommend imaging studies, such as an X-ray or MRI, to assess for any underlying injuries or conditions.

8. Physical Therapy: If the problem is related to a strain or overuse, a physical therapist can provide tailored exercises and treatments to aid recovery.

In conclusion, while there are many potential causes for swollen and painful ankles, understanding the specific context of your symptoms is essential for effective treatment. If you continue to experience discomfort or if the symptoms worsen, seeking medical advice is crucial to rule out any serious underlying conditions.
