Waking up with stiffness in the middle finger, ring finger, and little finger of both hands?
I recently heard my mother mention that after waking up, her middle finger, ring finger, and little finger are stiff for a while before returning to normal.
I wanted to ask about this.
She is 53 years old and has a habit of sleeping on her side (but regardless of which side she sleeps on, both hands become stiff).
It has been some time since the stiffness started.
Coincidentally, she also mentioned this because I recently had a hand strain, so I hope the doctor can provide some guidance.
Thank you.

Dr. Chen Wayda reply Rehabilitation
It is most likely a phenomenon of musculoskeletal degeneration.

Morning hand stiffness, particularly in the fingers, is a common complaint that can arise from various underlying conditions. In your mother's case, the stiffness in the middle finger, ring finger, and little finger upon waking could be attributed to several factors, including but not limited to, sleeping positions, potential nerve compression, or underlying inflammatory conditions.
Possible Causes of Morning Hand Stiffness:
1. Sleeping Position: As you mentioned, your mother has a habit of sleeping on her side. This position can sometimes lead to compression of nerves in the arm or wrist, particularly the ulnar nerve, which affects the ring and little fingers. Prolonged pressure on these nerves can result in temporary numbness, tingling, or stiffness upon waking.
2. Arthritis: Conditions such as osteoarthritis or rheumatoid arthritis can cause stiffness in the joints, especially in the morning. Rheumatoid arthritis, in particular, often presents with morning stiffness that lasts for more than an hour and can affect multiple joints symmetrically.
3. Tendonitis or Tenosynovitis: Inflammation of the tendons or the sheath surrounding the tendons in the fingers can lead to stiffness and pain. This is often exacerbated by repetitive motions or strain, which could be relevant if your mother has engaged in activities that stress her hands.
4. Carpal Tunnel Syndrome: This condition occurs when the median nerve, which runs through the wrist, becomes compressed. Symptoms often include numbness, tingling, and stiffness in the fingers, particularly in the morning.
5. De Quervain's Tenosynovitis: This condition affects the tendons on the thumb side of the wrist and can lead to pain and stiffness in the thumb and wrist, which may also radiate to the fingers.
Recommendations for Management:
1. Modify Sleeping Position: Encourage your mother to experiment with different sleeping positions or use a supportive pillow that keeps her arms in a more neutral position. She might also consider using a wrist brace at night to keep her wrist in a neutral position, reducing pressure on the nerves.
2. Gentle Stretching and Exercises: Once she wakes up, gentle stretching of the fingers and wrists can help alleviate stiffness. Simple exercises like flexing and extending the fingers, making a fist, and then opening the hand can promote circulation and reduce stiffness.
3. Heat Therapy: Applying a warm compress or soaking her hands in warm water can help relax stiff muscles and joints. Heat can improve blood flow and reduce discomfort.
4. Consult a Healthcare Professional: Given that your mother has experienced this stiffness for some time, it would be wise for her to consult a healthcare professional. A rheumatologist can evaluate for conditions like rheumatoid arthritis, while a neurologist can assess for nerve-related issues. Physical therapy may also be beneficial in developing a tailored exercise program.
5. Lifestyle Modifications: If your mother engages in repetitive hand movements, taking regular breaks and using ergonomic tools can help reduce strain on her hands.
6. Medication: If inflammation is suspected, over-the-counter anti-inflammatory medications like ibuprofen may provide relief. However, it’s essential to consult with a healthcare provider before starting any medication.
Conclusion:
Morning hand stiffness can be a multifactorial issue, and understanding the underlying cause is crucial for effective management. By addressing her sleeping position, incorporating gentle exercises, and seeking professional advice, your mother can work towards alleviating her symptoms. It's essential to monitor her condition and adjust her management plan as needed, especially if her symptoms persist or worsen.
