Brownish-red discharge - uterus has been removed?

Hello, my mother underwent a total abdominal hysterectomy and oophorectomy at the end of 2005, and it has been 7 years since then.
However, in the past three weeks, she has been experiencing intermittent reddish-brown discharge.
Last week, she went to the hospital for a Pap smear, and the result was "Grade 2 - benign inflammatory cells." I would like to ask why she is experiencing reddish-brown discharge.
Thank you.

Xiao PP, 30~39 year old female. Ask Date: 2013/03/19

Dr. Dong Yuhong reply Obstetrics and Gynecology

Hello: If there is reddish-brown discharge from the vagina, an internal examination should be conducted to confirm the presence of the reddish-brown discharge and to determine its cause.
Therefore, it is advisable for you to accompany your mother during her medical visit, as you can gain insights into the actual reasons for the issue through the physician's examination.
Thank you~

Understanding brownish discharge after a hysterectomy can be concerning, especially when it occurs several years post-surgery. In your mother's case, having undergone a total abdominal hysterectomy (TAH) and bilateral salpingo-oophorectomy (BSO) seven years ago, the appearance of brownish discharge warrants careful consideration of several potential causes.

1. Normal Post-Surgical Changes: After a hysterectomy, the body undergoes significant changes, including hormonal fluctuations, especially if the ovaries were removed. These hormonal changes can lead to various types of vaginal discharge, including brownish discharge, which may be old blood that is being expelled from the body. This can occur intermittently and may not necessarily indicate a problem.

2. Vaginal Atrophy: With the removal of the ovaries, your mother may experience vaginal atrophy due to decreased estrogen levels. This condition can lead to thinning of the vaginal walls, making them more susceptible to irritation and bleeding, which can manifest as brownish discharge.

3. Infection or Inflammation: The presence of "grade 2 - benign inflammatory cells" from the recent examination suggests that there may be some inflammatory process occurring. This could be due to a mild infection or irritation of the vaginal tissues. Infections can lead to abnormal discharge, and if there is any inflammation, it can also cause spotting or discharge that may appear brownish.

4. Endometrial Remnants: Although a total hysterectomy removes the uterus, in rare cases, remnants of endometrial tissue can remain and may respond to hormonal changes, leading to bleeding or discharge. This is less common but can occur, especially if there were any complications during the surgery.

5. Polyps or Other Growths: The development of vaginal or cervical polyps can also cause abnormal discharge. These growths are usually benign but can lead to spotting or discharge that may be brownish in color.

6. Other Considerations: Other factors such as trauma, foreign bodies, or even malignancies (though less likely given the time elapsed since surgery) should be considered, especially if the discharge is accompanied by other symptoms such as pain, itching, or a foul odor.

Given the complexity of the situation, it is crucial for your mother to follow up with her healthcare provider. They may recommend further evaluation, such as a pelvic examination, ultrasound, or possibly a biopsy if there are concerns about abnormal tissue growth. It is essential to monitor any additional symptoms, such as changes in the discharge's color, consistency, or odor, as well as any accompanying symptoms like pain or discomfort.

In summary, while brownish discharge after a hysterectomy can be due to various benign causes, it is essential to remain vigilant and consult a healthcare professional for a thorough evaluation to rule out any serious conditions. Regular follow-ups and open communication with her healthcare provider will help ensure her health and peace of mind.
