Cervical spine issues
Hello, doctor.
I would like to ask about my shoulder and neck pain.
I visited an orthopedic specialist who informed me that I have slippage and degeneration in cervical vertebrae 3, 4, and 5.
I have been undergoing rehabilitation, including traction, heat therapy, and electrical stimulation.
My weight is 80 kg, and I have managed to reduce it to 20 kg for some time, but I still experience shoulder and neck pain with limited improvement.
I have two questions: 1.
Is there a connection between my pillow and my shoulder and neck pain? I sleep on a relatively soft and low pillow; could this cause my cervical spine to tilt backward while I sleep, preventing adequate rest? 2.
If I need to change my pillow, are there any centers in Taiwan that conduct testing for pillows? Thank you.
Mr. Chen, 30~39 year old female. Ask Date: 2014/08/13
Dr. Chen Wayda reply Rehabilitation
An unsuitable pillow can lead to discomfort, but there are no pillow testing centers in Taiwan.

More Info
Understanding cervical spine issues, particularly in relation to neck pain and the impact of pillows, is crucial for effective rehabilitation and pain management. Based on your description, it seems you are experiencing significant discomfort due to cervical spondylosis and possible disc herniation at the C3-C5 levels. This condition can lead to nerve root impingement, resulting in pain, numbness, and weakness in the shoulders and arms.
1. The Relationship Between Pillows and Neck Pain
The type of pillow you use can significantly affect your neck's alignment during sleep. A pillow that is too soft or too low may not provide adequate support for the cervical spine, leading to improper alignment. When the neck is not properly supported, it can cause the cervical vertebrae to be in a flexed or extended position, which may exacerbate existing conditions like spondylosis or disc herniation.
For individuals with cervical spine issues, it is generally recommended to use a medium-firm pillow that maintains the natural curve of the neck. This helps to keep the spine aligned and can alleviate pressure on the cervical discs and nerve roots. A pillow that is too soft may allow the head to sink too deeply, causing the neck to bend unnaturally, while a pillow that is too firm may not contour to the neck's shape, leading to discomfort.
2. Pillow Testing Centers in Taiwan
As for your second question regarding testing centers for pillows in Taiwan, while there may not be specific centers dedicated solely to pillow testing, many orthopedic clinics and physical therapy centers offer assessments for sleep ergonomics. These assessments can help determine the best type of pillow for your specific needs based on your cervical spine condition. Additionally, some mattress and pillow retailers may provide trial periods or consultations to help you find a suitable product.
Additional Recommendations for Rehabilitation
In addition to addressing pillow choice, consider the following recommendations for managing your cervical spine issues:
- Physical Therapy: Continuing with physical therapy is essential. A physical therapist can provide tailored exercises that strengthen the neck and shoulder muscles, improve flexibility, and promote better posture.
- Ergonomic Adjustments: Evaluate your workstation and daily activities to ensure they are ergonomically sound. This includes adjusting the height of your chair, desk, and computer screen to reduce strain on your neck.
- Heat and Cold Therapy: Applying heat can help relax tight muscles, while cold therapy can reduce inflammation. Alternating between the two may provide relief.
- Medication Management: If you are experiencing persistent pain, discuss with your healthcare provider the possibility of adjusting your medication regimen. Medications such as NSAIDs or muscle relaxants may be beneficial.
- Mindfulness and Relaxation Techniques: Stress can exacerbate pain perception. Techniques such as mindfulness meditation, yoga, or deep breathing exercises can help manage stress and improve overall well-being.
Conclusion
In summary, the type of pillow you use can indeed impact your neck pain, especially with existing cervical spine issues. It is advisable to choose a pillow that supports proper alignment and to consult with healthcare professionals for personalized recommendations. Additionally, continuing rehabilitation efforts and making ergonomic adjustments can significantly contribute to your recovery and pain management. If symptoms persist, further evaluation by a specialist may be necessary to explore additional treatment options.
