Digestive issues
Hello, doctor.
In July of this year, I experienced digestive issues and consulted a gastroenterologist.
The doctor mentioned that my gastrointestinal tract was filled with gas, which was causing my indigestion.
I returned for follow-up visits twice, and the doctor prescribed medication, advising me to gradually reduce the frequency of use.
However, I felt it wasn't very effective, so I started taking a scientific herbal remedy called "Baohe Wan," which was initially very effective, but its effectiveness diminished over time.
Currently, I am taking "Bifidobacterium" three times a day, and I feel that my gastric digestion has improved, but my bowel movements occur only every two to four days.
I often experience flatulence and then feel reluctant to have a bowel movement.
I have a few questions:
1.
Is my current medication regimen appropriate? Is there room for improvement?
2.
Should I make any dietary adjustments?
3.
If my symptoms do not significantly improve, should I schedule another follow-up appointment?

Dr. Cai Anshun reply Gastroenterology and Hepatology
Hello: (1) It is recommended to return for a follow-up appointment.
(2) In terms of diet, it is advised to ensure adequate hydration, and to include sufficient vegetables and fruits, along with moderate exercise.
Additionally, developing a habit of regular bowel movements at the same time each day is important; the gastrocolic reflex is usually stronger in the morning (the presence of food in the stomach stimulates colonic motility).
It is recommended to wake up a bit earlier for breakfast and allow some time for using the restroom.
Once this habit is established, it will be less likely to experience fecal retention; the longer fecal matter remains, the drier it becomes, making it more difficult to pass.
(3) Regardless of whether traditional Chinese medicine or Western medicine is used, long-term use is not recommended; changing lifestyle and dietary habits is the key to long-term health.

More Info
Managing digestive issues can be quite challenging, especially when they stem from a combination of dietary habits, lifestyle choices, and underlying health conditions. Based on your description, it seems you have been experiencing digestive discomfort, including bloating and irregular bowel movements. Here are some recommendations and insights that may help improve your gut health.
1. Medication Use
You mentioned using "保和丸" (Bao He Wan) and "表飛銘" (Biao Fei Ming). While these traditional Chinese medicines can be effective for certain digestive issues, it's essential to monitor their effects closely. If you find that the initial benefits have diminished, it may be time to reassess their use. Medications can sometimes lose their effectiveness over time, or your body may adjust to them. If you continue to experience symptoms like bloating and infrequent bowel movements, it would be prudent to consult with your healthcare provider. They may suggest alternative medications or therapies that could be more effective for your current condition.
2. Dietary Adjustments
Diet plays a crucial role in managing digestive health. Here are some dietary adjustments you might consider:
- Increase Fiber Intake: A diet rich in fiber can help regulate bowel movements. Aim for a variety of fruits, vegetables, whole grains, and legumes. Foods like oats, beans, lentils, and leafy greens are excellent sources of fiber.
- Stay Hydrated: Drinking plenty of water is essential for digestion. It helps soften stool and promotes regular bowel movements. Aim for at least 8-10 cups of water daily, adjusting based on your activity level and climate.
- Limit Processed Foods: Processed foods can contribute to digestive issues. Try to minimize your intake of high-fat, high-sugar, and heavily processed foods, as they can exacerbate bloating and discomfort.
- Consider Food Sensitivities: Pay attention to how certain foods affect your digestion. Common culprits include dairy, gluten, and high-FODMAP foods (like certain beans, onions, and garlic). Keeping a food diary can help identify any patterns related to your symptoms.
- Regular Meal Times: Eating at consistent times can help regulate your digestive system. Try to have three balanced meals a day, with healthy snacks if needed, to keep your metabolism steady.
3. Lifestyle Modifications
In addition to dietary changes, consider the following lifestyle modifications:
- Regular Exercise: Physical activity can stimulate digestion and help prevent constipation. Aim for at least 30 minutes of moderate exercise most days of the week.
- Manage Stress: Stress can significantly impact gut health. Techniques such as mindfulness, yoga, or meditation can help reduce stress levels and improve digestion.
- Sleep Hygiene: Ensure you are getting adequate sleep, as poor sleep can affect digestive health. Aim for 7-9 hours of quality sleep each night.
4. When to Follow Up
If your symptoms persist despite making these changes, it is advisable to schedule a follow-up appointment with your healthcare provider. They may want to conduct further evaluations, such as blood tests, imaging studies, or even a colonoscopy, to rule out any underlying conditions that could be contributing to your digestive issues.
Conclusion
Improving gut health often requires a multifaceted approach that includes dietary changes, lifestyle modifications, and possibly medication adjustments. By focusing on a balanced diet rich in fiber, staying hydrated, managing stress, and maintaining regular physical activity, you can significantly enhance your digestive health. If symptoms persist, do not hesitate to seek further medical advice to ensure that any underlying issues are addressed. Remember, your gut health is a vital part of your overall well-being, and taking proactive steps can lead to significant improvements.
