Pain in the right thumb?
Hello Dr.
Chen: For the past 2 to 3 months, I have been experiencing aching pain on the palmar side of my right thumb.
The palmar aspect of the thumb joint (the fleshy part) often appears swollen.
Applying ice and heat reduces the swelling, but it swells again when I start doing household chores.
There is tenderness over the bones near the wrist at the thumb joint, and I also feel pain in the thumb joint itself.
Although I can still move it freely, the intermittent pain is quite bothersome.
Could this condition be related to a bone fracture or a dislocation/subluxation of the thumb? What should I do? Would continuous application of ice and heat help improve the situation? Due to this issue, I have been relying more on my left hand, and now I am starting to experience mild aching in the thumb joint of my left hand as well (without swelling).
I am really worried.

Hello, based on your condition, it could be due to overuse or early-stage osteoarthritis.
Please visit the outpatient clinic for further examination.
Thank you.

More Info
Understanding pain and swelling in the right thumb can be quite complex, especially when symptoms have persisted for a couple of months. Based on your description, it seems you are experiencing pain and swelling primarily in the palmar side of your right thumb, particularly around the metacarpophalangeal joint (the joint at the base of the thumb) and possibly extending towards the wrist.
Possible Causes
1. De Quervain's Tenosynovitis: This condition occurs when the tendons around the base of the thumb become irritated or inflamed. Symptoms often include pain and swelling at the base of the thumb, which can radiate up the forearm. Activities that involve repetitive wrist or thumb movements can exacerbate this condition.
2. Arthritis: Osteoarthritis or rheumatoid arthritis can lead to joint pain and swelling. In osteoarthritis, the cartilage that cushions the joints wears down over time, leading to pain, swelling, and decreased range of motion. Rheumatoid arthritis is an autoimmune condition that can also affect the joints, leading to similar symptoms.
3. Ligament Injury: A sprain or tear of the ligaments in the thumb can cause pain and swelling. This could happen due to a fall or an awkward movement. If there is significant pain with certain movements or if the thumb feels unstable, this could be a possibility.
4. Fracture or Dislocation: While you mentioned concerns about a possible fracture or dislocation, these typically present with more severe pain, swelling, and difficulty moving the thumb. If you had a recent injury that you suspect might have caused a fracture, it would be prudent to get an X-ray to rule this out.
5. Tendonitis: Inflammation of the tendons in the thumb can also lead to pain and swelling. This is often due to overuse or repetitive strain, especially if you have been doing more manual tasks than usual.
Management Strategies
1. Rest: Avoid activities that exacerbate the pain. If you find that certain movements or tasks worsen your symptoms, try to modify or avoid them.
2. Ice and Heat Therapy: Applying ice can help reduce swelling and numb the pain, especially after activities that aggravate your symptoms. Heat can be beneficial for relaxing and loosening tissues and stimulating blood flow to the area, but it should be used cautiously.
3. Splinting: A thumb spica splint can immobilize the thumb and provide support, which may help alleviate pain and allow the inflammation to subside.
4. Over-the-Counter Medications: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen can help reduce pain and inflammation.
5. Physical Therapy: A physical therapist can provide exercises to strengthen the muscles around the thumb and improve flexibility, which may help in recovery.
6. Consultation with a Specialist: If symptoms persist despite conservative management, it may be beneficial to consult with a hand specialist or an orthopedic surgeon. They can perform a thorough evaluation, possibly including imaging studies, to determine the exact cause of your symptoms and recommend appropriate treatment.
Conclusion
Given that you are experiencing ongoing pain and swelling in your right thumb, it is essential to address these symptoms proactively. While home management strategies can be effective, a thorough evaluation by a healthcare professional is crucial to rule out more serious conditions and to receive tailored treatment. If your left hand is also starting to show signs of discomfort, it may indicate compensatory strain due to overuse, which should also be addressed to prevent further issues.
