Hematuria one month after circumcision?
Hello, doctor.
I would like to ask a question.
It has been a month since I had a circumcision.
In the fourth week, the original blisters, scabs, and stitches have all fallen off, but I still experience swelling on both sides of the shaft when I wake up (the appearance looks more normal right after waking up).
The area is still quite sensitive, and I feel a soreness and occasional sharp pain when walking due to friction from my underwear.
Today, after having a bowel movement, I noticed some blood after urinating.
Normally, I would clean the area around the urethra with toilet paper after a bowel movement, but today there was a little bright red blood on the tissue (it did not mix with the urine, and it felt like there were small clots, but not solid).
It took about two or three wipes to clean it up, and I confirmed that it was not from the surgical site.
During urination, I did not feel much discomfort (only when I had a larger volume of urine did I feel some soreness near the urethra).
After that, I urinated two more times without any blood, but I still feel some soreness and sharpness on both sides of the shaft when walking.
In the second week, I had a similar experience where I bled after urinating too forcefully; at that time, the blood mixed with the urine and was noticeably pink when I wiped, and I felt a sharp pain near the urethra, but it stopped bleeding and hurting the next day.
I would like to know if this is normal and if I need to see a doctor for confirmation.
Also, I masturbated the night before; could that be related? Thank you.

Dr. Xu Weikai reply Urology
Xiao Lin: Hello, if there is bleeding from the wound, it is theoretically unrelated to the surgery.
There may be other issues causing blood in the urine.
I recommend seeking medical attention as soon as possible to check for conditions such as stones or infections.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

After undergoing circumcision, it is not uncommon to experience various sensations and symptoms as the body heals. In your case, noticing blood in the urine one month post-surgery can be concerning, but it is essential to understand the context and potential causes.
Firstly, the presence of blood in the urine, known as hematuria, can occur for several reasons, especially after a surgical procedure like circumcision. It is important to differentiate between blood that may be coming from the surgical site and blood that originates from the urinary tract. Since you mentioned that the blood was not mixed with urine and appeared after wiping, it is possible that the bleeding is related to irritation or trauma in the area around the urethra rather than from the bladder or kidneys.
The sensitivity and discomfort you are experiencing, particularly when walking or after certain activities like masturbation, could indicate that the area is still healing. The fact that you have experienced blood after urinating in the past, especially after straining, suggests that the urethra may be irritated or inflamed. This irritation can lead to minor bleeding, especially if there has been any trauma or excessive pressure applied to the area.
In terms of healing, it is normal for the surgical site to remain sensitive for several weeks, and some swelling can persist. The fact that you have not seen blood in subsequent urinations is a positive sign, but it is crucial to monitor the situation closely. If the bleeding recurs, or if you experience significant pain, swelling, or any other concerning symptoms, it would be wise to consult your healthcare provider. They can perform a thorough examination to rule out any complications such as infection, urinary tract issues, or other underlying conditions.
Regarding your question about whether masturbation could be related to the bleeding, it is possible. Engaging in sexual activity can sometimes cause irritation or trauma to sensitive areas, particularly if there is still some residual healing from the circumcision. If you notice that certain activities consistently lead to discomfort or bleeding, it may be beneficial to reduce those activities until you are fully healed.
In summary, while some bleeding can be normal after circumcision, especially if it is minor and infrequent, it is essential to remain vigilant. If you have any doubts or if the symptoms persist, seeking medical advice is the best course of action. Your healthcare provider can provide reassurance and ensure that your recovery is progressing as expected. Remember, healing takes time, and it is crucial to listen to your body during this period.
