Personality Issues: The Role of Serotonin and Medication - Psychiatry

Personality issues

Since childhood, I have been introverted, socially withdrawn, lacking in self-confidence, exhibiting perfectionistic tendencies, and prone to avoiding difficulties.
I often experience feelings of guilt, especially when faced with failures or setbacks, which leads me to easily feel disappointed in myself, doubt my abilities, and even question my worthiness of life.
I read that individuals with naturally low serotonin levels may experience such conditions; is this true? Additionally, I would like to inquire about the effects of two medications: ERISPAN and IMOVANE.
Thank you!

Andy, 30~39 year old female. Ask Date: 2000/11/01

Dear friend,
I apologize for the delay in my response due to issues with the hospital's email server.
It sounds like you are a sensitive person who is introspective and has high self-expectations.
You seek affirmation and approval for your actions, as this process helps you find value in your existence.
Such a personality may inevitably lead to emotional discomfort when faced with unfavorable situations.
Whether this personality trait is related to low serotonin levels is still a hypothesis and under research.
In reality, life is often filled with disappointments.
If everyone held themselves to such high standards, perhaps the world would be less chaotic, but it might also lead to a more constrained and restricted way of living.
My motto shared online is, "Authenticity is more important than perfection!" Life offers infinite possibilities and diversity; I cannot control every variable, but I can do my best within my limits.
Let us encourage each other in this regard!
Regarding the medications you inquired about, ERISPAN is an anxiolytic, while IMOVANE is a hypnotic.
Since I am unclear about what specific information you are seeking, I recommend consulting a psychiatrist if possible.
If you are already seeing a psychiatrist, please discuss your concerns with your attending physician.
Wishing you a peaceful holiday season and a happy New Year.
Sincerely,
Tang Xinbei

Understanding personality issues, particularly those related to introversion, low self-esteem, perfectionism, and avoidance tendencies, can indeed be complex and multifaceted. Your experiences of feeling guilty, especially in the face of failure, and questioning your self-worth are significant and deserve attention.

The Role of Serotonin
Serotonin is a neurotransmitter that plays a crucial role in regulating mood, anxiety, and overall emotional well-being. Research has shown that individuals with low levels of serotonin may be more susceptible to mood disorders, including depression and anxiety. This could explain why you feel a sense of low self-worth and experience heightened feelings of guilt and disappointment when faced with challenges.
Low serotonin levels can lead to symptoms such as sadness, irritability, and a lack of motivation, which can further exacerbate feelings of inadequacy and perfectionism. It is important to note that while serotonin levels can influence mood and personality traits, they are not the sole determinants. Genetics, environment, life experiences, and individual coping mechanisms also play significant roles in shaping personality and emotional responses.

Medication and Its Effects
Regarding the medications you mentioned, ERISPAN (which contains the active ingredient Escitalopram) and IMOVANE (which contains Zolpidem), both serve different purposes in the management of mood and anxiety disorders.

1. ERISPAN (Escitalopram): This medication is a selective serotonin reuptake inhibitor (SSRI) commonly prescribed for depression and anxiety disorders. By increasing the levels of serotonin in the brain, it can help improve mood, reduce feelings of anxiety, and enhance overall emotional stability. Patients often report feeling more balanced and less prone to the emotional swings that can accompany low serotonin levels. However, it is essential to be aware of potential side effects, which may include nausea, fatigue, insomnia, and sexual dysfunction.
2. IMOVANE (Zolpidem): This medication is primarily used for the short-term treatment of insomnia. It works by enhancing the effects of a neurotransmitter called gamma-aminobutyric acid (GABA), which promotes relaxation and sleep. While Zolpidem can be effective for sleep issues, it is not intended for long-term use due to the risk of dependence and tolerance. Side effects may include dizziness, daytime drowsiness, and in some cases, complex sleep behaviors (such as sleepwalking).

Considerations and Recommendations
Given your concerns about self-worth and emotional struggles, it may be beneficial to discuss your feelings and experiences with a mental health professional. Therapy, particularly cognitive-behavioral therapy (CBT), can help you develop healthier coping mechanisms, challenge negative thought patterns, and build self-esteem.
Additionally, if you are considering medication, it is crucial to have an open dialogue with your prescribing physician about your symptoms, concerns, and any side effects you may experience. They can help tailor a treatment plan that addresses both your emotional and psychological needs.

In summary, while serotonin plays a significant role in mood regulation, personality issues are complex and influenced by various factors. Medications like ERISPAN and IMOVANE can be helpful, but they should be part of a comprehensive treatment plan that includes therapy and lifestyle changes. Always consult with a healthcare professional before making any changes to your medication or treatment approach.
