Post-surgery issues after ankle surgery?

Hello, I would like to ask about my condition.
On January 13th, I had surgery to place two steel pins in my right ankle due to a minor fracture and displacement from a car accident.
Recently, I've noticed that the tendons behind my ankle feel very tight and tense, and I am unable to bend my foot.
Could it be that I have also injured my tendons?

Yuhan, 20~29 year old female. Ask Date: 2016/02/02

Dr. Shi Guozheng reply Orthopedics

After a fracture surgery, joint mobility may initially be less flexible, and it is necessary to actively move it up and down.
If the pain is manageable, generally, the tendons will not be injured, and with gradual practice, recovery typically occurs in about two months.

After undergoing surgery for a mild fracture and displacement of the right ankle on January 13, it is understandable to have concerns about tightness and mobility in the ankle. The presence of tightness in the muscles and tendons around the ankle can be a common post-surgical issue, especially after a procedure involving the insertion of hardware like screws. Here’s a detailed breakdown of your concerns and some additional insights into the recovery process.

1. Understanding Tightness and Pain: The tightness you are experiencing in the muscles and tendons behind your ankle could be due to several factors. Post-surgical inflammation is common and can lead to stiffness in the surrounding tissues. Additionally, immobilization during the healing process can cause the muscles to weaken and tighten, leading to a sensation of tightness or restriction in movement. It is also possible that the tightness is related to the healing of the ligaments and tendons that may have been affected during the injury or surgery.

2. Injury to Tendons or Ligaments: While it is possible that the tendons or ligaments around the ankle were injured during the accident, it is more likely that the tightness is a result of the surgical procedure and the subsequent healing process. If the tightness persists or worsens, it would be prudent to consult your orthopedic surgeon or a physical therapist. They can assess whether there is any significant injury to the tendons or ligaments and recommend appropriate treatment.

3. Rehabilitation and Mobility: Rehabilitation is crucial after ankle surgery. A physical therapist can guide you through exercises that promote mobility and flexibility in the ankle. These exercises typically start with gentle range-of-motion activities and gradually progress to strengthening exercises as healing allows. It is essential to follow a structured rehabilitation program to regain full function and prevent long-term stiffness.

4. Importance of Gradual Progression: It is important to be patient with your recovery. The timeline for regaining full mobility can vary based on the severity of the injury, the surgical procedure, and your overall health. Typically, patients can expect to see gradual improvements over weeks to months. If you experience significant pain during exercises, it may indicate that you are pushing too hard, and you should adjust your activity level accordingly.

5. Consulting Your Surgeon: If you have concerns about the tightness or if it is accompanied by other symptoms such as swelling, redness, or increased pain, it is essential to consult your orthopedic surgeon. They may recommend imaging studies, such as an X-ray or MRI, to ensure that there are no complications from the surgery, such as hardware issues or additional injuries.

6. Long-term Outlook: Most patients who undergo surgery for ankle fractures can expect to regain full function with appropriate rehabilitation. However, some may experience lingering tightness or discomfort, especially if they do not engage in a comprehensive rehabilitation program. Staying active and following your therapist's recommendations will be key to a successful recovery.

In summary, the tightness you are experiencing is likely a common post-surgical issue, but it is essential to monitor your symptoms and engage in a structured rehabilitation program. If you have any concerns about your recovery, do not hesitate to reach out to your healthcare provider for personalized advice and support.
