Persistent Health Issues: A 26-Year-Old's Journey - Family Medicine

Sick for seven months?

I am a 26-year-old male, and since March of this year, I have been experiencing a series of health issues.
Initially, I had unexplained gas pressure on my chest, followed by changes in my bowel habits, now having a bowel movement once or twice a day (previously once every three days).
My stools are shaped but very soft, with colors mostly being light yellow and occasionally dark green, almost black.
Additionally, I have developed issues with my nose; the bridge of my nose often feels tight, leading to difficulty breathing and requiring me to breathe through my mouth.
My hands, feet, and face also experience numbness.
I have undergone several tests, including gastroscopy, colonoscopy, nasal endoscopy, chest X-ray, and blood tests, all of which returned normal results, except for the discovery of seven polyps in my colon that were removed.
What could be wrong with me? Please help me, and what further tests can I undergo?

Dear Mr.
Yin: You have undergone various examinations related to your symptoms, such as gastroscopy, colonoscopy, nasal endoscopy, chest X-ray, and blood tests, all of which have been confirmed as normal.
The colon polyps have been removed.
The next step may be to visit a psychiatrist at a mental health facility, which may be beneficial for you.
Our hospital has excellent psychiatrists, and you are welcome to come for a consultation.

It sounds like you have been going through a challenging time with your health, and I understand how concerning it can be when multiple symptoms arise without a clear diagnosis. Based on the symptoms you've described—persistent gas pressure in the chest, changes in bowel habits, unusual stool colors, nasal congestion, and episodes of numbness in your extremities—there are several potential avenues to explore further.

Firstly, it's important to recognize that gastrointestinal issues can often manifest in various ways, including changes in bowel habits and gas. The presence of polyps in your colon, while concerning, is not uncommon, especially in younger individuals. These polyps can sometimes lead to changes in bowel habits and stool consistency. The fact that you had them removed is a positive step, as it reduces the risk of potential complications.

The gas pressure in your chest could be related to gastrointestinal issues, such as gastroesophageal reflux disease (GERD) or functional dyspepsia, which can cause discomfort that mimics cardiac symptoms. It's crucial to differentiate between gastrointestinal and cardiac issues, especially since you mentioned experiencing difficulty breathing and numbness. While your tests have come back normal, it may be beneficial to consult with a gastroenterologist for a more in-depth evaluation of your digestive system.

The changes in your stool color—particularly the presence of light yellow and dark green or near-black stools—can indicate various conditions. Light-colored stools can sometimes suggest a lack of bile, which may be due to liver or gallbladder issues, while dark stools can indicate bleeding in the upper gastrointestinal tract. Given that your tests have returned normal, it may be worth discussing these specific changes with your healthcare provider to rule out any underlying conditions.

The nasal congestion and difficulty breathing could be related to allergies or sinus issues, which can also cause discomfort and lead to mouth breathing. If you have not already done so, consider seeing an ear, nose, and throat (ENT) specialist to evaluate your nasal passages and sinuses for any potential blockages or infections.

The numbness in your hands, feet, and face is particularly concerning and warrants further investigation. This could be related to nerve compression, anxiety, or even a neurological condition. It would be advisable to consult a neurologist who can perform a thorough evaluation, including nerve conduction studies or imaging if necessary.

In summary, while your initial tests have returned normal, the persistence of your symptoms suggests that further evaluation is warranted. Here are some recommendations:
1. Gastroenterology Follow-Up: Discuss your gastrointestinal symptoms in detail with a gastroenterologist, focusing on the changes in bowel habits and stool color.

2. ENT Consultation: Consider seeing an ENT specialist for your nasal congestion and breathing difficulties.

3. Neurological Evaluation: A neurologist can assess the numbness and tingling in your extremities and face, which may require additional testing.

4. Lifestyle Modifications: Keep a symptom diary to track your symptoms, dietary habits, and any potential triggers. This information can be invaluable for your healthcare providers.

5. Mental Health Support: Given the stress of ongoing health issues, consider speaking with a mental health professional who can help you manage anxiety and stress related to your health concerns.

Navigating health issues can be overwhelming, but with the right support and thorough evaluations, you can work towards finding answers and improving your well-being.
