Is sinusitis recurrent?
Hello Dr.
Tsai: I had sinus surgery a little over two years ago, and now I am experiencing post-nasal drip again.
Is it possible that I have a recurrence of the condition? Is post-nasal drip also referred to as nasal drip? Thank you for your response.

Dr. Cai Zhenggu reply Otolaryngology
Mr.
Yun: If it is thick post-nasal drip, it may be a recurrence of sinusitis.
Post-nasal drip is different from nasal discharge.

Hello! Thank you for your question regarding the recurrence of sinusitis and the symptoms you are experiencing post-surgery. It’s not uncommon for patients who have undergone sinus surgery to have concerns about the potential for recurrence of sinusitis, especially if they are experiencing symptoms like post-nasal drip, which is indeed often referred to as "nasal drip" or "post-nasal drip."
To begin with, let's clarify what sinusitis is. Sinusitis, or inflammation of the sinuses, can be acute or chronic. Acute sinusitis typically lasts for a short duration, often following a cold or respiratory infection, while chronic sinusitis can persist for months or even years, leading to ongoing symptoms. Post-surgical patients may experience a range of symptoms as their sinuses heal, and it is essential to differentiate between normal post-operative symptoms and signs of a potential recurrence of sinusitis.
Post-nasal drip occurs when excess mucus accumulates in the back of the throat, which can be a result of sinus inflammation or infection. After sinus surgery, it is common for patients to experience some degree of mucus production as the sinuses heal. However, if you notice a significant increase in mucus, especially if it is thick, discolored, or accompanied by other symptoms such as facial pain, fever, or a decrease in your sense of smell, it may indicate that you are experiencing a recurrence of sinusitis.
Several factors can contribute to the recurrence of sinusitis after surgery. These include:
1. Anatomical Issues: Sometimes, the underlying anatomical issues that contributed to the initial sinusitis may not be fully resolved by surgery. For example, if there are structural abnormalities in the nasal passages or sinuses, they may lead to blockages that can cause inflammation and infection.
2. Allergies: Allergic reactions can lead to inflammation of the nasal passages and sinuses, which can mimic or exacerbate sinusitis symptoms. If you have a history of allergies, it may be worth exploring this aspect further.
3. Environmental Factors: Exposure to irritants such as smoke, pollution, or strong odors can also trigger sinus inflammation.
4. Infections: Viral infections, such as the common cold, can lead to sinus inflammation and potentially result in a recurrence of sinusitis.
5. Nasal Polyps: These are non-cancerous growths that can develop in the nasal passages and sinuses, leading to blockage and inflammation. If you had nasal polyps previously, they may recur after surgery.
If you suspect that your symptoms are indicative of a recurrence of sinusitis, it is crucial to consult with your ENT specialist. They may perform a physical examination, possibly including nasal endoscopy, to assess the condition of your sinuses and determine if there is any blockage or infection present. Imaging studies, such as a CT scan, may also be utilized to provide a clearer picture of your sinus anatomy and any potential issues.
In summary, while post-nasal drip can be a normal part of the healing process after sinus surgery, it is essential to monitor your symptoms closely. If you experience worsening symptoms or new concerns, please reach out to your healthcare provider for a thorough evaluation. Early intervention can help manage symptoms effectively and prevent further complications. Wishing you a smooth recovery and good health!
