Post-Surgery Concerns: Pain and Issues After Circumcision in Urology - Urology

Postoperative care

It has been over two months since my circumcision surgery, and I would like to ask why my glans does not fully emerge during an erection.
Additionally, retracting the glans causes pain, especially during an erection, where it feels very tight as if something is constricting it.
When the glans is exposed during an erection, it is quite painful.
Therefore, when I masturbate, I can only do so without retracting the glans to avoid pain; otherwise, retracting it makes masturbation impossible.
I would like to ask the doctor if this indicates a failure of the surgery.

It sounds like there may be an issue.
It is advisable to return for a follow-up appointment to have the surgeon confirm the situation.

After undergoing circumcision, experiencing pain and discomfort, particularly during erections, is not uncommon. However, the symptoms you describe—such as the inability of the glans (the head of the penis) to fully emerge, significant pain when it does, and a feeling of tightness—are concerning and warrant further investigation.

Firstly, it is essential to understand that circumcision is a surgical procedure that involves the removal of the foreskin, which can lead to changes in sensitivity and the way the skin of the penis behaves during erections. Post-operative complications can include scarring, excessive tightening of the skin, or even the formation of adhesions, which can cause the glans to feel trapped or restricted. This could explain the pain you experience when the glans is exposed during an erection.

The sensation of tightness and pain during erections could indicate a condition known as "phimosis," which is the inability to retract the foreskin fully. Although you have undergone circumcision, if there is excessive scarring or if the remaining skin is too tight, it can mimic phimosis symptoms. This can lead to discomfort during sexual activity, as you have described.

Another possibility is that you may be experiencing a condition called "penile hypersensitivity," which can occur after circumcision. The glans becomes more exposed and sensitive, leading to discomfort or pain during erections or sexual activity. This heightened sensitivity can make the experience of sexual arousal and penetration painful.

To address your concerns, it is crucial to consult with a urologist or a healthcare professional specializing in male reproductive health. They can perform a physical examination to assess the surgical site and determine if there are any complications such as scarring or tightness that may require intervention. In some cases, a minor surgical procedure may be necessary to release any tight skin or adhesions that are causing discomfort.

In the meantime, here are some recommendations that may help alleviate your symptoms:
1. Gentle Stretching: If comfortable, you may try gentle stretching of the skin around the glans to help reduce tightness. However, be cautious and stop if it causes pain.

2. Lubrication: Using a water-based lubricant during sexual activity may help reduce friction and discomfort.

3. Avoiding Irritants: Ensure that any soaps or products used in the genital area are mild and free from irritants that could exacerbate sensitivity.

4. Pain Management: Over-the-counter pain relief medications, such as ibuprofen or acetaminophen, may help manage discomfort, but consult with your healthcare provider before taking any medication.

5. Follow-Up Appointment: Schedule a follow-up appointment with your urologist as soon as possible to discuss your symptoms in detail. They may recommend imaging studies or further evaluations to rule out any underlying issues.

In conclusion, while some discomfort after circumcision is normal, the severity and nature of your symptoms suggest that further evaluation is necessary. Early intervention can help address any complications and improve your quality of life. Don't hesitate to seek medical advice, as your health and comfort are paramount.
