Nervous urination leading to accidental ejaculation?

I am a middle school student and have never masturbated.
Everything has been normal until one day when the bell rang for class, and I needed to urinate but couldn't be late.
I panicked and ran to the restroom, feeling very nervous.
On the way, I noticed some urine leaking out, and when I got to the restroom and pulled down my pants, I realized it was white, which should be semen.
That was my first ejaculation.

A few days later, I had a dream where I was taking an exam, and when it was time to collect the papers, I hadn't finished.
Although it was just a dream, I felt very anxious, and I experienced the same sensation as when I rushed to the restroom a few days prior.
When I ejaculated in the dream, I woke up to find my underwear damp.
Since then, I often feel the urge to urinate, and even after I go, I still feel the need to urinate.

Is it possible that this is nocturnal emission or is there some other issue? I found online that frequent urination might have psychological factors, as I never experience nocturia.
At that time, I was preparing for a major exam, so I thought I would wait until after the exam to see if it improved.
However, now that the exam is over, I still frequently feel the urge to urinate, although the intense anxiety leading to ejaculation has diminished.
Thank you, doctor.

Dr. Sun Hongshun reply Urology

Hello: The occurrence of nocturnal emissions at your age is normal and may be related to stress, exams, or fear of being reprimanded.
It is recommended to relax and engage in more physical activity.
If the symptoms persist, please follow up at a urology clinic.
Don't worry too much!

Premature ejaculation (PE) and urinary urgency are two distinct but potentially interrelated issues that can affect teenagers, particularly during periods of stress or anxiety. Your experience, as described, highlights the interplay between psychological factors and physiological responses, which is quite common during adolescence.

Understanding Premature Ejaculation:
Premature ejaculation is characterized by ejaculation that occurs sooner than desired, often with minimal sexual stimulation. In your case, the incident where you felt a sudden urge to urinate and subsequently experienced ejaculation could be linked to heightened anxiety. The body’s response to stress can trigger various physiological reactions, including increased heart rate, muscle tension, and changes in hormonal levels, which can affect sexual function.
During adolescence, hormonal changes are significant, and the body is still maturing. It’s not uncommon for young males to experience spontaneous ejaculation, especially during moments of excitement or anxiety, such as rushing to class. This can be a normal part of sexual development, and many young men experience similar occurrences.

Urinary Urgency:
Urinary urgency, or the sudden and strong need to urinate, can also be influenced by psychological factors. In your situation, the stress of potentially being late for class may have heightened your awareness of bodily sensations, leading to a feeling of urgency. This is often referred to as "psychogenic urgency," where the mind influences the body’s response to urination.
The fact that you do not experience nocturnal enuresis (bedwetting) suggests that your bladder function is generally normal during sleep, which is a positive sign. However, the persistent feeling of needing to urinate, even after you have just gone, can be distressing. This could be due to a variety of factors, including anxiety, bladder sensitivity, or even habits formed during stressful periods.

Psychological Factors:
It’s essential to recognize that psychological stress can significantly impact both urinary function and sexual health. The anxiety associated with exams or performance can lead to increased muscle tension and hyper-awareness of bodily sensations, which may exacerbate feelings of urgency or lead to premature ejaculation.
Recommendations:
1. Relaxation Techniques: Consider incorporating relaxation techniques such as deep breathing, meditation, or mindfulness exercises into your routine, especially during stressful periods like exams. These can help manage anxiety and reduce the physical symptoms associated with it.

2. Consult a Healthcare Professional: If the feelings of urgency persist or if you have concerns about your sexual health, it may be beneficial to speak with a healthcare provider. They can help assess whether there are any underlying medical issues and provide guidance tailored to your situation.

3. Education and Awareness: Understanding that these experiences are common among adolescents can help alleviate some of the anxiety associated with them. Many young men go through similar phases, and it’s a normal part of development.

4. Healthy Lifestyle: Maintaining a healthy lifestyle, including regular exercise, a balanced diet, and adequate hydration, can positively impact both urinary health and overall well-being.

In conclusion, your experiences with premature ejaculation and urinary urgency are likely influenced by a combination of physiological and psychological factors. While it can be concerning, these issues are often temporary and can be managed with the right strategies and support. If symptoms persist or worsen, seeking professional advice is always a wise step.
