Knee contusion?
Hello doctor, I was in a car accident two weeks ago, and I had a laceration on my knee that required two stitches.
The stitches were removed after 10 days, and the external wound looks mostly healed with scabs formed.
However, I am unable to fully bend my knee, and I feel a tightness, although there is no pain.
There is a slight swelling in the knee, but it is not very noticeable.
Could this be due to the wound not being fully healed, or is it related to something inside the knee? Thank you!

A knee contusion may only be a skin laceration, but if there is significant swelling, it could indicate internal bleeding in the joint.
You can try applying a warm compress to see if it reduces swelling or alleviates tightness.
Delayed healing of the wound is also a contributing factor.
The swelling from a contusion typically takes about two weeks to subside.
If pain persists or if there are issues with bending the knee after that time, it is advisable to consult a physician.

After a car accident, it's not uncommon to experience lingering issues with your knee, even if the external wounds appear to be healing well. Given your description, it seems that while the superficial injuries have started to heal, there may be underlying issues that are affecting your knee's range of motion and causing the tightness you feel.
Firstly, the inability to fully bend your knee and the sensation of tightness could be attributed to several factors. One possibility is that the soft tissues surrounding the knee, such as ligaments, tendons, and muscles, may have sustained some degree of strain or injury during the accident. Even if X-rays show no fractures, soft tissue injuries can lead to swelling and stiffness, which can impede your ability to bend the knee fully.
The swelling you mentioned, although not pronounced, could be indicative of fluid accumulation in the joint, known as effusion. This can occur due to inflammation from the injury, and it can contribute to the sensation of tightness and restricted movement. It's important to note that the healing process for soft tissues can take longer than for skin wounds, often requiring several weeks to months for full recovery.
Another consideration is the possibility of a meniscus injury or damage to the cartilage within the knee joint. The meniscus acts as a cushion and stabilizer for the knee, and injuries to this structure can lead to pain, swelling, and limited range of motion. If there was any twisting or impact during the accident, it could have resulted in such an injury, even if it wasn't immediately apparent.
In terms of recovery, it's crucial to engage in a proper rehabilitation program. Physical therapy can be highly beneficial in restoring range of motion and strength to the knee. A physical therapist can guide you through exercises tailored to your specific condition, focusing on improving flexibility, strength, and stability. They may also employ modalities such as ice, heat, or electrical stimulation to help reduce swelling and promote healing.
It's also essential to listen to your body during this recovery phase. If you experience pain or significant discomfort while attempting to bend your knee, it's advisable to stop and consult with a healthcare professional. They may recommend imaging studies, such as an MRI, to assess for any internal injuries that may not have been detected initially.
In summary, while your external wounds may be healing, the tightness and limited range of motion in your knee could be due to soft tissue injuries, swelling, or potential internal damage. Engaging in a structured rehabilitation program and consulting with a healthcare provider for further evaluation will be key to ensuring a full recovery. Remember, patience is vital in the healing process, and it's essential to give your body the time it needs to heal properly.
