Drug side effects
Hello, Dr.
Hsu: I have been experiencing sleep issues for about two years.
My psychiatrist has prescribed me Olanzapine (30 mg), Escitalopram (10 mg), and Zolpidem (7.5 mg), and I am currently tapering the dosage, following instructions to take 1/4, 1/2, and 1 full tablet.
I have plans for family planning with my girlfriend, but I can only ejaculate during self-stimulation and not during intercourse.
I consulted with my psychiatrist, but since he is not a urologist, he was uncertain whether it could be related to the medications (possibly Escitalopram).
I would like to ask if the aforementioned medications have side effects that could affect ejaculation during intercourse? If not, what tests or treatments should I consider? Thank you, doctor.
Xiao Chen, 20~29 year old female. Ask Date: 2018/04/13
Dr. Xu Wencang reply Urology
Escitalopram is an SSRI that can cause delayed ejaculation.
The primary treatment should focus on psychiatric care, and the medication should be gradually tapered off.
If there are any issues, please seek consultation at the outpatient clinic.

Medications can indeed have a significant impact on sexual function, including ejaculation. In your case, the medications you are taking—specifically Olanzapine (often referred to as Zyprexa), Escitalopram (often referred to as Lexapro), and possibly the sleep aid you mentioned—can contribute to sexual side effects, including difficulties with ejaculation.
Olanzapine is an atypical antipsychotic that is often used to treat conditions such as schizophrenia and bipolar disorder. One of the known side effects of Olanzapine is sexual dysfunction, which can manifest as decreased libido, erectile dysfunction, and difficulties with ejaculation. This is due to its effects on neurotransmitter systems, particularly dopamine and serotonin, which play crucial roles in sexual arousal and function.
Escitalopram, on the other hand, is a selective serotonin reuptake inhibitor (SSRI) commonly prescribed for depression and anxiety. SSRIs are well-documented to cause sexual side effects, including delayed ejaculation or anorgasmia (the inability to achieve orgasm). This is thought to be related to the increased serotonin levels in the brain, which can inhibit sexual function.
The sleep medication you mentioned, while less commonly associated with sexual side effects, can also contribute to overall sedation and may indirectly affect sexual performance and function.
Given your situation, it is plausible that the combination of these medications could be contributing to your difficulty with ejaculation during intercourse. It is important to discuss these concerns with your prescribing physician, as they may consider adjusting your medication regimen. This could involve switching to a different antidepressant with a lower risk of sexual side effects, such as bupropion, or adjusting the dosage of your current medications.
In addition to medication review, it may be beneficial to explore other factors that could be influencing your sexual function. Stress, anxiety, and relationship dynamics can all play significant roles in sexual performance. Engaging in open communication with your partner and possibly seeking counseling or therapy could be beneficial.
If your physician determines that the medications are indeed the cause, they may suggest a few options:
1. Medication Adjustment: This could involve switching to a different class of medications that have a lower incidence of sexual side effects.
2. Supplemental Treatments: In some cases, medications like sildenafil (Viagra) or tadalafil (Cialis) may be prescribed to help with erectile function, which can sometimes improve overall sexual satisfaction.
3. Therapy: Cognitive-behavioral therapy (CBT) or sex therapy can help address psychological factors that may be contributing to sexual dysfunction.
4. Lifestyle Changes: Incorporating regular exercise, a healthy diet, and stress management techniques can also improve overall sexual health.
In summary, it is essential to have an open dialogue with your healthcare provider about your concerns regarding ejaculation and sexual function. They can help you navigate the complexities of medication side effects and explore alternative treatments or strategies to enhance your sexual health.
