Left lower abdominal pain in specific positions?
Hello Doctor, since March, after one run, I started experiencing pain in my lower left abdomen the next day.
After resting for a week, the pain subsided, but it returned when I resumed running.
This cycle continued for a month.
In April, I visited a urologist (I had a hernia as a child), and after examination, the doctor stated it was not a hernia but noted mild varicocele.
Other tests were normal, and surgery was not recommended.
The doctor advised me to avoid activities that cause pain and to rest for a while, as the cause of the left lower abdominal pain could not be determined (it does not hurt during normal activities, only after running or in specific positions/situations).
After several months of testing, I found that the following situations cause left lower abdominal pain.
Could you please help evaluate the possible reasons or suggest which specialty I should consult? Thank you.
1.
Lying flat and raising my legs for abdominal exercises causes intense pain in the lower left abdomen.
2.
In a lunge position with my right foot forward and left foot back (without bending the knee), exerting force with my right foot backward and my left foot forward results in pain in the lower left abdomen.
3.
Jogging 6-8 km (at a pace of about 6'15"): I may not experience symptoms immediately after the first run, but usually, after the second run, I start to feel a sense of heaviness or persistent discomfort, requiring 2-3 days of rest afterward.
4.
Hiking (to Wuliaojian in Sanxia): I do not feel anything after a 3-hour hike, but after exceeding three hours, I experience noticeable discomfort with each step.
5.
Generally, going up and down stairs or normal jumping does not cause any sensation.

Dr. Wang Zhaomin reply Rehabilitation
Abdominal muscle strain, abdominal wall hernia, and intestinal issues can all potentially lead to left lower abdominal pain.
In addition to symptom description, a physical examination or relevant special tests are necessary for accurate diagnosis.
Online consultations cannot replace an actual medical visit.
It is advisable to consult a general surgeon, urologist, orthopedic specialist, or rehabilitation physician for further evaluation.

More Info
Understanding left lower abdominal pain during specific activities can be quite complex, as it may stem from various underlying causes. Based on your description, it seems that the pain is activity-specific, particularly during exercises that engage the abdominal muscles or involve certain movements. Here are some potential explanations and considerations for your symptoms.
Possible Causes of Left Lower Abdominal Pain
1. Muscle Strain or Overuse: Given that the pain occurs during specific activities such as leg raises or lunges, it is possible that you are experiencing a muscle strain. This could be due to overuse or improper form during exercises, leading to strain in the abdominal or hip flexor muscles.
2. Inguinal or Sports Hernia: Although your previous examination ruled out a hernia, it is worth noting that some hernias can be subtle and may not be easily detected during initial evaluations. A sports hernia, which is a soft tissue injury in the groin area, could also cause pain during physical activities.
3. Nerve Entrapment: The pain could be related to nerve entrapment, such as the ilioinguinal or iliohypogastric nerve, which can cause localized pain in the lower abdomen during certain movements.
4. Genitourinary Issues: Given your history of mild varicocele, it is essential to consider that issues related to the reproductive system, such as inflammation or irritation, could manifest as abdominal pain during physical exertion.
5. Visceral Pain: Sometimes, pain in the abdomen can be referred from other organs, such as the intestines or kidneys. Conditions like diverticulitis or even mild gastrointestinal disturbances could cause discomfort that is exacerbated by physical activity.
6. Pelvic Floor Dysfunction: If you are engaging in activities that put pressure on the pelvic floor, such as running or certain abdominal exercises, it could lead to discomfort in the lower abdomen.
Recommendations for Further Evaluation
1. Consult a Sports Medicine Specialist: Since your symptoms are closely related to physical activities, a sports medicine physician can provide a thorough evaluation. They can assess your exercise technique and may recommend imaging studies, such as an MRI or ultrasound, to rule out any structural issues.
2. Physical Therapy: Engaging in physical therapy can help strengthen the core muscles and improve flexibility, which may alleviate pain during specific activities. A physical therapist can also provide guidance on proper exercise techniques to prevent further injury.
3. Pain Management: If the pain persists, consider discussing pain management options with your healthcare provider. This may include anti-inflammatory medications or other modalities to help manage discomfort.
4. Activity Modification: While you are undergoing evaluation, it may be beneficial to modify your activities. Avoid exercises that exacerbate the pain and focus on low-impact activities that do not trigger symptoms.
5. Follow-Up with Urology: Since you have a history of mild varicocele, it may be worth following up with a urologist to ensure that there are no underlying issues contributing to your symptoms.
Conclusion
In summary, left lower abdominal pain during specific activities can arise from various causes, including muscle strain, nerve entrapment, or even issues related to the genitourinary system. A comprehensive evaluation by a sports medicine specialist, along with physical therapy, may help identify the underlying cause and develop an appropriate treatment plan. It is essential to listen to your body and avoid activities that trigger pain while seeking professional guidance.
